| dc.description.abstract | The city of Governador Valadares is in Minas Gerais, and belongs to the middle region of Vale do Rio Doce and micro eponymous city. On the outskirts of town is the Jardim Primavera neighborhood, where it operates the team ESF Jardim Primavera that was developed this work, which is justified by the high prevalence of hypertension among the population of the Jardim Primavera community, the number of hypertensive patients with blood pressure uncontrolled, aiming to develop a proposal for educational and dynamic intervention on critical nodes of hypertension for people of this community. For the development of this project we used the simplified method of Situational Strategic Planning - SSP held meeting with staff and a review of literature on the subject, using databases Scielo, Lilacs and Medline, considering publications since 1998. It follows that identifying the problems and the "critical nodes", it was possible to create a plan of action, with the design of operations, identification of critical resources, plan feasibility analysis and development of operational plan, aimed at tackling the problem, establishing actions improving the quality of life, increasing people's knowledge about the disease and the correct use of medications with distribution of organizational boxes for medications, and stimulating change in lifestyle through physical activity groups, to combat alcoholism and smoking. | |
