Diabetes is considered an important risk factor for complications that occur in the lower extremities. The objectives of this study were to describe the evaluation process of the feet of diabetes patients cared for in primary care, according to analysis of protocols, recognizing the risk factors that can be modified by stimulating self-care, along with an appropriate metabolic control, which therefore reduce the risk of ulcers and amputations and develop a systematic clinical evaluation roadmap for the organization of care for the diabetic foot in Conselheiro Lafaiete, MG. And while recognizing the risk factors that can be modified by stimulating self-care, along with an appropriate metabolic control, which consequently reduce the risk of ulcers and amputations. The methodology used consisted of an integrative review of scientific literature available on the virtual network and university libraries, based on the following terms: primary care, comprehensive care and diabetes, epidemiology of diabetes; teamwork in diabetes management, nursing consultation in control diabetes, diabetic foot care in primary care and Health Family. The results allowed identifying the main interventions of low complexity that can contribute to the prevention of ulcers, minimizing the influence of risk and therefore the number of amputations. We stress the importance of attending to family health team for people with diabetes, because the synergy of their actions is essential to promote adaptation to chronic health condition and get to practicing self-care. The reflections made in this study helped contribute to the elaboration of a guide for evaluation of people with diabetes, with emphasis on examination and foot care, I want to deploy on my team, after reaching a consensus of all involved. It is suggested rethinking needs to formulate a training program for professionals related health and primary care, in order to have a quality service and greater efficiency in the care provided to diabetic patients.
