Diabetes Mellitus: estratégia para melhor abordagem ao paciente

It is known that diabetes is a clinical syndrome characterized by chronic hyperglycemia with disturbances in the metabolism of carbohydrates, lipids and proteins, with high mortality, especially when not properly monitored. In the population served in our coverage area condition is worth mentioning, considering that the team records point to underdiagnosis of the disease, considering the national epidemiology. The records related to the status of compensation patients are also quite precarious and most importantly, monitoring the same does not follow any protocol being done almost by spontaneous demand. This research aimed to develop an action plan to improve the monitoring of diabetic patients in the Family Health Strategy Tia Lia in Mendonça and Veredinha, Minas Gerais. Overlooking the grounds of the Plan became bibliographical research in the SciELO database, the virtual library NESCON and the Guidelines of the Brazilian Diabetes Society acquis, with the descriptors: Diabetes Mellitus, Pre-diabetic State, Complications of Diabetes, Diabetic Retinopathy, Risk Factors and Continuity of patient care. The intervention project was structured based on Situational Strategic Planning (SSP) method and after put in place, there is an improvement in the quality of care for diabetic patients is expected, based on a standard model of monitoring, with shares scheduled and consequently improving the indicators of the disease.
