Ameloblastoma is a benign odontogenic tumor, slow growing, but aggressive behavior due to histological patterns that tend to infiltrate through the trabeculated bone adjacent to the lesion, which explains the high recurrence rates. Many treatments are proposed, which include more conservative approaches such as curettage and marsupialization to the most radical, with partial resection or hemimandibulectomy, with a margin of safety. This paper presents a case of recurrent ameloblastoma in soft tissue, after 15 years of mandibular resection with vascularized costo-chondral graft and discusses the occurrence of recurrence, planning and treatment possibilities. It is essential a proper diagnosis associating radiographic and tomographic images with the clinical history and, regardless of the conduct, the postoperative control for years is essential because of the aggressive and recurrent character of the injury.
