dc.description.abstractThe objective of this study was to develop an Action Plan for the development of an interdisciplinary practice by the Family Health Team of Hope Neighborhood 1, the city of Caratinga, Minas Gerais. The study was guided by narrative review, covering the topics: interdisciplinary work, team work, health promotion, physical activity and health promotion. Later, he became the Building Action Plan, based on the methodology proposed by the module Planning and Evaluation of Health Actions of the specialization in Primary Care Family Health at the Federal University of Minas Gerais. So, it was possible to build an Action Plan with a view to coping with some obstacles that have hindered interdisciplinary practice within the family health team's Hope Neighborhood 1, in Caratinga. For problems on the lack of memory and record of decisions has created the "Schedule of success"; regarding the lack of communication, the Project was prepared "Who does not communicate, will get burned" and to the lack of education processes permanent originated Constant Education Project.
