Femoroacetabular impingement (FAI) produce lesions in labrum and acetabular cartilage and is a known risk factor for development of hip arthrosis. Arthroscopy emerged as a minimally invasive method to treat FAI. There is, however, great variability in the literature regarding its indications and results. Part of this variability is related to comparison of patients with labral and chondral lesions at different stages of severity. In order to reduce this variability patients with lesions classified in similar stages should be compared. It is expected to find in this study the classifications of labral and chondral lesions with arthroscopic surgery that present greater reproducibility. Consequently, communication, treatment outcome, and prognostic evaluation can be more reliable. The reliability of the labral and chondral classifications most commonly described in the literature were evaluated using the Kappa weighted values. The Lage, Seldes and Beck classifications for acetabular labrum lesions present good interobserver reliability. In the intraobserver evaluation the classifications of Lage and Seldes presented good reliability and Beck classification showed excellent reliability. The Outerbridge, Beck and Haddad classifications showed good interobserver reproducibility and excellent intraobserver reproducibility when evaluating arthroscopic acetabular chondral lesions.
