Doença peri-implantar em indivíduos com e sem manutenção preventiva: um follow-up de 5 anos

To determine and compare the incidence of peri-implantitis in individuals with mucositis in a 5 years follow-up study. In addition, determine with periodontal and peri-implant parameters the influence of preventive maintenance. 212 partially edentulous individuals, rehabilitated with dental implants had accomplished a periodontal and peri-implant clinical examination in 2005 (BL). Five years later, 80 individuals with diagnosis of mucositis in the BL examination were re-examined in a follow-up study (FE). These individuals were divided in two groups: GTP group (n=39) that had accomplished preventive maintenance within 5-years study period, and GNTP group (n=41) without preventive maintenance. The following parameters were clinically evaluated: Plaque Índex (PI), bleeding on periodontal probing (BP) and peri-implant probing (BPi), periodontal (PD) and peri-implant (PDI) probing depth, supuration and peri-implant bone-loss. The influence of biological and behavioural risk variables associates to the peri-implantitis occurrence was evaluated by means of univaried and multivaried analysis of logistic regression. The incidence of peri-implantitis in the global sample was 31.2%, being 43.9% for GNTP and 18.0% for GTP group. Worse peri-implantitis condition (plaque index, % of places with BPi, % of places with PDi 5 mm, number of implants with peri-implantitis and peri-implant bone-loss (p<0.05).) was reported in GNTP group. Multivaried analysis of logistic regression significantly associates the following variables to peri-implantitis: GTP group (>50% of places with BPi; OR=37,27 and > 5% of places with PDp 4mm; OR=25.49 and a OR=11.43 of presence of periodontitis. The absence of preventive maintenance, in individuals with peri-implant mucositis mainly, had presented high incidence of peri-implantitis. Clinical parameters as BPi, PDp and presence of periodontitis were strongly associates to a higher risk to develop peri-implantitis.
