Systemic arterial hypertension is a multifactorial clinical condition characterized by elevated and sustained blood pressure levels and is associated with metabolic alterations, with a consequent increase in the risk of fatal and non-fatal cardiovascular events. The situational diagnosis based on the Situational Strategic Planning method identified, as the main problem for coping, the high prevalence of patients with systemic arterial hypertension. Thus, this study aimed to develop an intervention plan to reduce the incidence of arterial hypertension in patients attending the Petrolândia II Family Health Program in the city of Contagem-Minas Gerais. A bibliographic research was also carried out in the Virtual Health Library with the descriptors: hypertension, risk factors, health education, and primary health care. Through the construction and implementation of this intervention plan, we can conclude that the primary health care professionals' performance is of great importance to contribute to improving the quality of life of each individual in our community.
