dc.description.abstractIn Brazil, arterial hypertension is an important risk factor for cardiovascular outcomes. Cardiovascular diseases are important causes of morbidity and mortality and generate high economic costs, which increase progressively with an increase in blood pressure. Adequate control of patients with arterial hypertension should be one of the priorities of Primary Care, based on the premise that early diagnosis, proper management and appropriate treatment of hypertension are essential for the reduction of adverse cardiovascular events. The objective of this work is to elaborate an intervention plan addressing the issue of Arterial Hypertension and its factors, aiming to reduce the morbimortality of the disease and to achieve a greater control and prevention of it in the PSF Alecrim in the city of São Gonçalo do Rio Preto / Minas Gerais. To prepare the intervention proposal for hypertensive patients enrolled in the team, three stages were performed: situational diagnosis, bibliographical review and elaboration of the action plan, using Situational Strategic Planning. We used scientific papers available in a database such as: Virtual Health Library, Virtual Library of the Federal University of Minas Gerais, Scientific Electronic Library Online - SciELO. The causes of the problems, selected as critical nodes, were: poor eating habits and inadequate lifestyles, social pressure that generates stress, patient information level to improve knowledge about their illness, receive and attend the patient, solving the problem better and lastly the work process of the health team.
