We conducted a systematic review to investigate the effects of exercise on physical, functional and cognitive capacity of institutionalized older adults with dementia. Searches were performed on Medline, PEDro, Lilacs, Scielo and Cocrhane Library. This review follows the PRISMA guidelines. Only randomized controlled trials were included. We evaluated the studies quality by PEDro scale and the strength of the current evidence using the GRADE approach. A total of ten studies matched the criteria, with a sample of 876 participants with mean age of 80 years old. The interventions were diversified, with multicomponent exercises, high intensity functional training and activities of daily living training. The duration varied from three to six months and frequency averaged two to three times a week. Exercise improved physical, functional and cognitive capacity, but there’s no consistent evidence of the best exercise intervention for the best effects. Studies with better methodological quality should be encouraged.
