The myocardial infarction is caused by an interruption of blood flow in the coronary arteries that supply the myocardium, leading to a tissue necrosis causing serious damage to the heart muscle. It has a high incidence and severity, and is considered a major health problem that requires immediate attention. One of indicated treatment is thrombolysis, which promotes the degradation of fibrin clots and provides the return of local blood flow. It is a procedure performed very often in European countries and in the United States, in units of prehospital care by doctors or paramedics trained. This study aims to identify the benefits of thrombolytic therapy use in patients afflicted with acute myocardial infarction in the prehospital environment. In this study was used evidence-based practice and integrative review, the research was done in databases: LILACS, MEDLINE, IBECS and SciELO. There are 06 studies from European countries, published in English language and authored medical. The results shows that the main benefits of the use of thrombolytics in the pre-hospital are mortality reduction, improved levels of reperfusion, lower intrahospitalar complication rates and lower cost. It is concluded that prehospital thrombolysis has a lot of importance and efficiency and can be used in Brazil since there was training professionals for the rapid diagnosis of infarction, performing electrocardiogram in pre-hospital setting and decision making agile and assertive.
