dc.description.abstract | Introducción: Los Síndromes Mielodisplásicos (SMD) son un grupo de neoplasias hematológicas heterogéneas que se caracterizan por hematopoyesis ineficaz. Se manifiestan con displasia en los diferentes linajes celulares y citopenias en sangre periférica [1]. La mediana de edad en el momento del diagnóstico de SMD es de 70 años y <10% tienen menos de 50 años. La incidencia de SMD es de aproximadamente 4 casos /100.000 habitantes / año (alcanzando 40-50 / 100.000 en pacientes de 70 años) [2]. En Colombia no se tienen datos a nivel nacional que detallen las características de SMD. El objetivo del presente estudio es establecer la caracterización de la población de pacientes con Síndrome Mielodisplásico atendidos en la Foscal de 2013 a 2019.
Materiales y Métodos: Se realizó un análisis secundario de una base de datos, es un estudio de cohorte retrospectivo y prospectivo en un centro de referencia de tercer nivel en Colombia con 83 pacientes que fueron diagnosticados con SMD entre junio de 2013 y junio de 2019. La mayoría de los pacientes fueron manejados con base en el protocolo las Guías Españolas para SMD [5]. Métodos estadísticos descriptivos y de resumen fueron usados para la caracterización de los pacientes y Kaplan-Meier se utilizó para evaluar la supervivencia global y las tasas de supervivencia libre de recaída en un año, tres y cinco años.
Resultados: Los pacientes presentaron una edad con rango entre 32 y 104 años
al momento del diagnóstico, con una mediana de 76 (RIQ 70 a 82)Únicamente 10 pacientes (12,3%) eran menores de 60 años. Cuarenta y cinco eran hombres (54,2%). El tipo más común fue el SMD sin clasificar (SMD-U) 39,51%, seguida de Citopenia refractaria con displasia multilinaje - RCMD (34,57%) y Anemia refractaria con exceso de blastos-RAEB-2 (14,81%). Diecinueve (22,9%) pacientes se les realizó estudios citogenéticos con la detección de del(5q) como la anormalidad más común en 4 pacientes (4.8%), seguido por la del(7q) in 2 pacientes (2,4%). Siete de cada diez pacientes recibieron soporte transfusional con UGRE. Recibieron Azacitidina 35%de los pacientes y 4,8% Decitabina. Dos (2,45%) pacientes recibieron trasplante de progenitores hematopoyéticas (TPH) alogénico. La supervivencia global al año fue de 68.2% (CI95% 56,1-77,6) y de 33,7% (CI95% 20,2-47,7) a los cinco años. La supervivencia global para los que recibieron TPH fue de 100%, mientras para los que recibieron Azacitidina para el primer año 68% (IC95% 47,4-81,9) y para cinco años fue de 21,3% (IC95% 6,1-41,4). Se identificaron 42 pacientes (52,5%) fallecidos con 15 (40,5%) muertes debido a progresión de la enfermedad. En Veintiséis pacientes (32%) no fue posible establecer seguimiento.
Conclusiones: La supervivencia global de SMD fue muy similar a la informada en la literatura existente en nuestro medio. La primera causa más común de muerte en los pacientes con SMD fue la infección, seguida por la progresión de la enfermedad primaria. | |
