In the present chapter we describe some of the important basic details that are involved in completing a Far Lateral Transcondylar Approach for Lesions of the Foramen Magnum. The anatomy relevant to this approach was examined by some of the authors (Evandro de Oliveira, Hung Tzu Wen and Helder Tedeschi) in cadaveric specimens and thoroughly described. The surgical applications and pitfalls observed during a series of clinical cases are also discussed.
