Resumen
Frailty and anemia in the elderly appear to share a common pathophysiology associated with chronic inflammatory processes. This study uses an analytical, cross-sectional, population-based methodology to investigate the probable relationships between frailty, red blood cell parameters and inflammatory markers in 255 community-dwelling elders aged 65 years or older. The frailty phenotype was assessed by non-intentional weight loss, fatigue, low grip strength, low energy expenditure and reduced gait speed. Blood sample analyses were performed to determine hemoglobin level, hematocrit and reticulocyte count, as well as the inflammatory variables IL-6, IL-1ra and hsCRP. In the first multivariate analysis (model I), considering only the erythroid parameters, Hb concentration was a significant variable for both general frailty status and weight loss: a 1.0. g/dL drop in serum Hb concentration represented a 2.02-fold increase (CI 1.12-3.63) in an individual's chance of being frail. In the second analysis (model II), which also included inflammatory cytokine levels, hsCRP was independently selected as a significant variable. Each additional year of age represented a 1.21-fold increase in the chance of being frail, and each 1-unit increase in serum hsCRP represented a 3.64-fold increase in the chance of having the frailty phenotype. In model II reticulocyte counts were associated with weight loss and reduced metabolic expenditure criteria. Our findings suggest that reduced Hb concentration, reduced RetAbs count and elevated serum hsCRP levels should be considered components of frailty, which in turn is correlated with sarcopenia, as evidenced by weight loss. 59 3 636 641 Artz, A.S., Anemia and the frail elderly (2008) Seminars in Hematology, 45, pp. 261-266 Barzilay, J.I., Blaum, C., Moore, T., Xue, Q.L., Hirsch, C.H., Walston, J.D., Insulin resistance and inflammation as precursors of frailty: The Cardiovascular Health Study (2007) Archives of Internal Medicine, 167, pp. 635-641 Beghe, C., Wilson, A., Ershler, W.B., Prevalence and outcomes of anemia in geriatrics: A systematic review of the literature (2004) American Journal of Medicine, 116, pp. 3S-10S Bross, M.H., Soch, K., Smith-Knuppel, T., Anemia in older persons (2010) American Family Physician, 82, pp. 480-487 Cesari, M., Leeuwenburgh, C., Lauretani, F., Onder, G., Bandinelli, S., Maraldi, C., Frailty syndrome and skeletal muscle: Results from the Invecchiare in Chianti study (2006) American Journal of Clinical Nutrition, 83, pp. 1142-1148 Cesari, M., Penninx, B.W., Pahor, M., Lauretani, F., Corsi, A.M., Rhys Williams, G., Inflammatory markers and physical performance in older persons: The InCHIANTI study (2004) Journals of Gerontology, 59, pp. 242-248 Chang, S.S., Weiss, C.O., Xue, Q.L., Fried, L.P., Association between inflammatory-related disease burden and frailty: Results from the Women's Health and Aging Studies (WHAS) I and II (2012) Archives of Gerontology and Geriatrics, 54, pp. 9-15 Chen, P.C., Sung, F.C., Chien, K.L., Hsu, H.C., Su, T.C., Lee, Y.T., Red blood cell distribution width and risk of cardiovascular events and mortality in a community cohort in Taiwan (2010) American Journal of Epidemiology, 171, pp. 214-220 Denny, S.D., Kuchibhatla, M.N., Cohen, H.J., Impact of anemia on mortality, cognition, and function in community-dwelling elderly (2006) American Journal of Medicine, 119, pp. 327-334 Ershler, W.B., Biological interactions of aging and anemia: A focus on cytokines (2003) Journal of the American Geriatrics Society, 51, pp. S18-S21 Fattori, A., Santimaria, M.R., Alves, R.M., Guariento, M.E., Neri, A.L., Influence of blood pressure profile on frailty phenotype in community-dwelling elders in Brazil - FIBRA study (2013) Archives of Gerontology and Geriatrics, 56, pp. 343-349 Ferrucci, L., Balducci, L., Anemia of aging: The role of chronic inflammation and cancer (2008) Seminars in Hematology, 45, pp. 242-249 Folstein, M.F., Folstein, S.E., McHugh, P.R., "Mini-mental state."
