In order to evaluate evoked potentials in the assessment of neuropsychiatric manifestations (NPM) in systemic lupus erythematosus (SLE), 27 unselected patients were analysed by clinical and laboratory methods: neurological evaluation included neuropsychological assessment using the Mini-mental State Examination (MMSE) and evaluation of somatosensory, brainstem auditory and visual evoked potentials, as well as cerebral computerized tomography (CCT). Seventy percent of the patients had NPM and 37% had neurologic disturbances, mainly a defective short-term memory that was significantly associated with seizures. Cerebral atrophy detected by CCT was present in 44% of the patients but there was no correlation with evoked potentials, NPM manifestations or neuropsychological abnormality tests. Somatosensory evoked potentials were the most frequent neurophysiologic alteration but were only observed in patients with neurological symptoms or deficits. Alterations in the visual evoked potentials occurred in four patients without visual symptoms and were reversible. Brainstem auditory alterations occurred in only one uremic patient. Evoked potentials can be used in the management of neuropsychiatric lupic patients, to evaluate somatosensory, visual or auditive pathways.
