dc.descriptionThe authors report the results of 20 cases of gastric ulcers, submitted to gastrectomy with pylorus preservation, over a period of 3 yr, following the modified techniques of Maki et al (1967). The purpose of this technique is to avoid the dumping syndrome, caused by elimination of the pylorus sphincter. For this, a section is made 1.5 cm above the pylorus, resecting 50% of the stomach and renewing passage by gastrogastric anastomosis similar to Billroth I, at a level of extramucous seromuscular suture with an overlap of the edges, with separate stitches of cotton 30 thread. The dumping syndrome did not occur in any of the patients. Systematic postoperative roentgenographic studies showed no gastric stasis. Basal secretion values and maximum responses to hystamine were in most cases lower than at the preoperative stage. The authors registered one recurrence of anastomotic mouth ulcerations and one case of stenosis of the anastomosis area through a prolapse of the antro residual mucosa. Both required additional surgery.
