Resumen
Cyclosporine, a drug used in immunosuppression protocols for hematopoietic stem cell transplantation that has a narrow therapeutic index, may cause various adverse reactions, including nephrotoxicity. This has a direct clinical impact on the patient. This study aimsto summarize available evidence in the scientific literature on the use of cyclosporine inrespect to its risk factor for the development of nephrotoxicity in patients submitted to hematopoietic stem cell transplantation. A systematic review was made with the following electronic databases: PubMed, Web of Science, Embase, Scopus, CINAHL, LILACS, SciELO and Cochrane BVS. The keywords used were: “bone marrow transplantation” OR “stem cell transplantation” OR “grafting, bone marrow” AND cyclosporine OR cyclosporin OR “risk factors” AND “acute kidney injury” OR “acute kidney injuries” OR “acute renal failure” OR “acuterenal failures” OR “nephrotoxicity”. The level of scientific evidence of the studies was classified according to the Oxford Centre for Evidence Based Medicine. The final sample was composed of 19 studies, most of which (89.5%) had an observational design, evidence level 2B and pointed to an incidence of nephrotoxicity above 30%. The available evidence, considered as good quality and appropriate for the analyzed event, indicates that cyclosporinere presents a risk factor for the occurrence of nephrotoxicity, particularly when combinedwith amphotericin B or aminoglycosides, agents commonly used in hematopoietic stem cell transplantation recipients. 36 5 363 368 Borel, J.F., Feurer, C., Gubler, H.U., Stähelin, H., Biological effects of cyclosporin A: A new antilymphocytic agent (1976) Agents Actions, 6, pp. 468-475 Yee, G.C., Pharmacokinetic and pharmacodynamic studies of cyclosporine in bone marrow transplantation (1990) Transplant Proc, 22, pp. 1327-1330 Leather, H.L., Drug interactions in the hematopoietic stem cell transplant (HSCT) recipient: What every transplanter needs to know (2004) Bone Marrow Transplant, 33, pp. 137-152 Kahan, B.D., Keown, P., Levy, G.A., Johnston, A., Therapeutic drug monitoring of immunosuppressant drugs in clinical practice (2002) Clin Ther, 24, pp. 330-350. , discussion 329 Yee, G.C., Clinical pharmacology of cyclosporine (1990) Int J Rad Appl Instrum B, 17, pp. 729-732 Lopes, J.A., Jorge, S., Acute kidney injury following HCT: Incidence, risk factors and outcome (2011) Bone Marrow Transplant, 46, pp. 1399-1408 Hosing, C., Nash, R., McSweeney, P., Mineishi, S., Seibold, J., Griffith, L.M., Acute kidney injury in patients with systemic sclerosis participating in hematopoietic cell transplantation trials in the United States (2011) Biol Blood Marrow Transplant, 17, pp. 674-681 Parikh, C.R., Schrier, R.W., Storer, B., Diaconescu, R., Sorror, M.L., Maris, M.B., Comparison of ARF after myeloablative and nonmyeloablative hematopoietic cell transplantation (2005) Am J Kidney Dis, 45, pp. 502-509 Hingorani, S.R., Guthrie, K., Batchelder, A., Schoch, G., Aboulhosn, N., Manchion, J., Acute renal failure after myeloablative hematopoietic cell transplant: Incidence and risk factors (2005) Kidney Int, 67, pp. 272-277 Kersting, S., Koomans, H.A., Hene, R.J., Verdonck, L.F., Acute renal failure after allogeneic myeloablative stem cell transplantation: Retrospective analysis of incidence, risk factors and survival (2007) Bone Marrow Transplant, 39, pp. 359-365 Parikh, C.R., Coca, S.G., Acute renal failure in hematopoietic cell transplantation (2006) Kidney Int, 69, pp. 430-435 da Costa Santos, C.M., de Mattos Pimenta, C.A., Nobre, M.R., The PICO strategy for the research question construction and evidence search (2007) Rev Lat Am Enfermagem, 15, pp. 508-511 Thompson Reuters, , https://www.myendnoteweb.com/EndNoteWeb.html?returnCode=ROUTER.Unauthorized&SrcApp=CR&Init=Yes, Endnote.
