dc.description.abstractIn theareas of Emergency, Hospitalization and Outpatient of Pharmacy Service in theHospital of IESS in Ambato, diagnosis and evaluation of drug delivery system wasperformed. At the beginning an observation sheet was applied in different areas ofpharmacy randomly to some auxiliary staff where the parameters of good dispensingdetermined that the parameters do not met the required standards, as well as a survey to15 individuals which determined that the 60% has an empirical average knowledge ondrugs. Regarding the factor that influences the most for the occurrence of dispensingerrors is the wrong diagnosis by the doctor and the dispensing error by the assistant. Inthe research in the three areas a total of 668 errors were found by Mc Connell andBarker´s method of observation which is used for the identification and quantification ofmedication errors. The first point is the severity of the error, being the most frequent typeA; followed by the type of error by its nature which is the wrong medicine and thepredominant is therapeutic duplication. To know the origin of faults, it was studied thecauses being the main human factor by doctors especially for not checking themedication previously sent. And the service with the highest number of errors is InternalMedicine. Therefore, it is necessary to inform those responsible for achieving preventmedication errors. It is recommend to apply the Distribution System for Unit DoseMedication pursuant to Ministerial Agreement 2744, integrating biochemical-pharmacists to be part of the health team to provide comprehensive and quality healthservices.
