The Pharmaceutical Intervention in the elderly patient with Cerebrovascular Disease of the Neurology Service was carried out at the Instituto Ecuatoriano de Seguridad Social "IESS" of Riobamba. It was applied to 30 patients who voluntarily accepted to be part of the research, with the objective of detecting drug-related problems and negative results associated with medication. Applying the Dáder Method, 90% of the patients were identified as having drug-related problems and 10% did not. The distribution of the DRPs was 48% with non-quantitative effectiveness, 32% with non-quantitative safety, 10% related to untreated health problems, 6% with safety and 3% with quantitative effectiveness of the medication. Pharmaceutical interventions (FI) were carried out, corresponding to 62.5 FI/patient, 22.5% Pharmacist-Physician, with a predominance of verbal communication between pharmacist and patient. The results obtained allow us to conclude that the SFT is a useful instrument at hospital level, since by means of the statistical analysis of Student's T, it was confirmed that the calculated T was higher for the critical T, showing significant differences in the parameters of blood pressure, cholesterol and triglycerides, It was possible to reduce and control these parameters in the patients and thus improve the quality of life of the older adult. It is recommended to provide the Pharmaceutical Care service, with the Health education program to the relatives or caregivers of patients with cerebrovascular accident, to prevent a second event from occurring.
