Orientador: Renata Cunha Matheus Rodrigues Garcia Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: A artrite reumatoide (AR) é uma doença prevalente que pode causar danos severos à articulação temporomandibular (ATM) e, possivelmente, às funções do sistema estomatognático. O estudo teve como objetivo comparar a função mastigatória de idosos com e sem AR e comprometimento da ATM. Para isso, 30 voluntários com idade igual ou acima de 60 anos, parcial ou totalmente desdentados foram selecionados em dois grupos: (1) com AR e DTM, (2) sem AR e sem DTM (controle). O comprometimento da ATM foi avaliado através do RDC/TMD. Os voluntários foram reabilitados por meio de próteses removíveis e avaliados, antes e após a instalação das novas próteses, quanto às variáveis: força máxima de mordida (FMM), performance mastigatória (PM), movimentos mastigatórios, amplitudes máximas de movimentos mandibulares e impacto da saúde oral na qualidade de vida (ISOQV). A FMM foi determinada por meio de sensores de pressão colocados bilateralmente na região dos molares. A PM foi avaliada por método de fracionamento em peneiras, utilizando material teste, considerando o tamanho mediano de partículas (X50) e a distribuição das partículas ao longo das peneiras (b). Traçados eletromagnéticos da trajetória mandibular foram utilizados para avaliar os movimentos mastigatórios (tempo, angulação e velocidade) durante a mastigação de material teste e as amplitudes máximas dos movimentos mandibulares (abertura sagital e vertical, lateralidade direita e esquerda, desvio lateral mandibular e protrusão). O ISOQV foi avaliado pelo OHIP-14 e seus domínios envolvendo aspectos físicos, pscicológicos e sociais. Os dados foram analisados por ANOVA e teste de Tukey-Kramer, com nível de significância de ?=0,05. Observou-se que idosos com AR+DTM apresentaram significativa redução na FMM (P<0,0001), no ângulo de abertura dos movimentos mastigatórios e nas amplitudes dos movimentos de abertura sagital e vertical, lateralidade direita e esquerda, assim como apresentaram valores significativamente maiores de X50 e b, indicando baixa PM (P <0,05). Também se observou maior ISOQV negativo nos aspectos gerais e domínios (D1, D2, D3, D4, D5 e D7), no grupo com AR+DTM com relação ao grupo controle. Após a reabilitação protética, houve aumento significativo em ambos os grupos (P <0,05), na FMM, na distribuição das partículas (menores valores de b), no ângulo de abertura e fechamento dos movimentos mastigatórios, e nas amplitudes de movimentos mandibulares, exceto para desvio lateral (P = 0,3769) e lateralidade direita (P = 0,0735). Embora o grupo AR+DTM manteve os menores valores para as variáveis avaliadas, as novas próteses tiveram maior efeito positivo na função mastigatória desse grupo, refletido na redução significante do X50 durante a PM e redução do ISOQV negativo nos sete domínios (P <0,05). No grupo controle ocorreu redução do ISOQV negativo em geral e domínios D1 e D2. Conclui-se que o comprometimento da ATM em idosos com AR pode prejudicar a função mastigatória, causando maior ISOQV negativo, quando comparado à idosos sem a doença. E próteses bem planejadas mostraram-se cruciais no tratamento odontológico dos idosos com a doença, pois ajudam a minimizar os efeitos deletérios da AR na função mastigatória, assim como podem beneficiar a mastigação em idosos em geral diminuindo o ISOQV Abstract: Rheumatoid arthritis (RA) is a prevalent disease, which may cause severe damage to the temporomandibular joint (TMJ) and possibly to the stomatognathic system functions. The aim of this study was to compare the masticatory function of elderly individuals with and without TMJ commitment due to RA. For this, 30 volunteers aged over 60 years, partially or totally edentulous, were assigned to two groups: (1) with RA and TMJ impaired, (2) without RA or TMD (control). The TMJ was evaluated by the RDC/TMD. Subjects were rehabilitated through new removable prostheses, and the elderly were evaluated before and after prosthesis installation for the following variables: maximum bite force (MBF), masticatory performance (MP), jaw chewing movements, range of mandibular movements and oral health-related quality of life (OHRQoL). The MBF was determined via pressure sensors placed bilaterally in the molar region. The MP was evaluated by sieving method using test material, considering the median particle size (X50) and distribution of particles along the sieve (b). An electromagnetic jaw tracking method was used to evaluate jaw chewing movements (time, angle and velocity) during mastication of the test material, and the range of mandibular movements (sagittal and vertical opening, right and left laterality, mandibular lateral deviation and protrusion). The OHRQoL was assessed by OHIP-14 and their physical, psychological and social domains. The data obtained were analyzed using ANOVA followed by Tukey-Kramer test. All tests were performed at a significance level of ? = 0.05. It was observed that elderly individuals with RA and TMJ impairment showed a significant reduction in the MBF (P <0.0001), the opening angle of the jaw chewing movements and the range of mandibular movements of the sagittal and vertical opening, and right and left laterality, as well as a significant increase in X50 and b values, indicating low MP (P <0.05). It also found greater negative OHRQoL in general aspects and domains (D1, D2, D3, D4, D5 and D7), in the RA+TMD group when compared to the control. After prosthesis installation, there was a significant increase in both groups (P <0.05) in MBF, distribution of the particles (lower b values), the opening and closing angles of the jaw chewing movements and the range of mandibular movements, except for lateral deviation (P = 0.3769) and right laterality (P = 0.0735). Although the RA+TMD group maintained the lowest values, the new prosthesis had a greater positive effect on the masticatory function in this group, reflected in significant reduction in X50 during the MP, in the OHRQoL, and all domains (P <0.05). In the control group, there was a reduction in the negative OHRQoL in general and domains D1 and D2. Therefore, it was concluded that the commitment in TMJ due to AR impairs masticatory function among the elderly, causing a negative OHRQoL.
