Ilustraciones spa:Objetivo: Establecer la asociación entre mortalidad a 5 años y factores de riesgo presentes en pacientes con cáncer de cérvix localmente avanzado en una institución de tercer nivel del departamento de Caldas entre los años 2010 y 2017. Materiales y métodos: Estudio observacional, analítico, transversal, retrospectivo. Se identifico diferencias de distribución de las variables según el estadio vital a 5 años aplicando chi cuadrado de homogeneidad, U de Mann – Whitney y t de student según fue el caso. Se realizó regresión logística múltiple, considerando variables estadísticamente significativas las que presentaran OR con p <0.05 e intervalos de confianza del 95% que no cruzó el valor nulo. Resultados: Se analizaron 185 pacientes. Se observó diferencias significativas en edad, estadio figo, necesidad de transfusión, falla renal que contraindica el inicio de la quimioterapia, karnosfky, grado histológico, tipo de tratamiento, uso de quimioterapia concurrente y uso de radioterapia de haz externo. El grado histológico (OR= 1.92, p 0.015, IC95%: 1.13 – 3.24), la necesidad de transfusión (OR= 2.32, p 0.026, IC95%: 1.10 –4.86) y la falla renal que contraindico el inicio de quimioterapia (OR= 8.13, p 0.001, IC95%: 2.37 – 29.05) se comportaron como factores de riesgo independiente para mortalidad a 5 años. Conclusiones: El grado histológico, la necesidad de trasfusión y la falla renal que contraindico el inicio de quimioterapia se comportaron como factores de riesgo independientes para mortalidad a 5 años en pacientes con cáncer de cérvix localmente avanzado eng:Objective: To establish the association between 5-year mortality and risk factors present in patients with locally advanced cervical cancer in a tertiary institution in the department of Caldas between 2010 and 2017. Materials and methods: Observational, analytical, cross-sectional, retrospective study. Differences in the distribution of variables according to the vital stage at 5 years were identified by applying chi-square of homogeneity, Mann-Whitney U and Student's t-test as appropriate. Multiple logistic regression was performed, considering statistically significant variables those presenting OR with p <0.05 and 95% confidence intervals that did not cross the null value. Results: 185 patients were analyzed. Significant differences were observed in age, figo stage, need for transfusion, renal failure contraindicating the initiation of chemotherapy, karnosfky, histologic grade, type of treatment, use of concurrent chemotherapy and use of external beam radiotherapy. Histological grade (OR= 1.92, p 0.015, 95%CI: 1.13 - 3.24), transfusion requirement (OR= 2.32, p 0.026, 95%CI: 1.10 -4.86) and renal failure contraindicating initiation of chemotherapy (OR= 8.13, p 0.001, 95%CI: 2.37 - 29.05) behaved as independent risk factors for 5-year mortality. Conclusions: Histological grade, need for transfusion and renal failure contraindicating the initiation of chemotherapy behaved as independent risk factors for 5-year mortality in patients with locally advanced cervical cancer. 1.
