355 p. Recurso Electr?nico Esta tesis problematiza la educaci?n m?dica que se ofrece en los programas de
pregrado de medicina en Colombia, y sus efectos en la relaci?n m?dico?paciente,
lo cual es asumido como una pr?ctica discursiva susceptible de ser estudiada,
haciendo uso de las herramientas propuestas por Michel Foucault.
Parti? de la pregunta por la emergencia de m?ltiples transformaciones
enunciativas que ha tenido la relaci?n m?dico-paciente en las ?ltimas d?cadas,
y la incidencia de las pr?cticas gubernamentales de la educaci?n m?dica, en la
emergencia de cierto tipo de sujeto, que desde sus diversas posiciones da lugar
a otros ?nfasis y modos de funcionamiento de la relaci?n m?dico-paciente en
Colombia, en el per?odo de tiempo comprendido entre 1992 y 2014.
La delimitaci?n temporal, sit?a el inicio de la decada del noventa, como el
momento en que Colombia, en el marco de la Conferencia de Promoci?n de la
Salud y Equidad, realizada en Bogot? en 1992 (Organizaci?n Panamericana de
la Salud, Organizaci?n Mundial de la Salud, 1992), adopta las recomendaciones
sugeridas para alcanzar la meta ?Salud para todos en el a?o 2000?, propuesta
durante la Conferencia Internacional de Atenci?n Primaria en Salud de Salud, en
Alma?Ata, Kazajistan, (Organizaci?n Mundial de la Salud, 1978), y ratificada en
1986 durante la Primera Conferencia Internacional para la Promoci?n de la
Salud, que tuvo lugar en Otawa, Canad? (Organizaci?n Panamericana de la
Salud, 1986). En lo que tiene que ver con la educaci?n m?dica, estas
recomendaciones fueron precisadas a trav?s de los mecanismos de control y
gesti?n, establecidos por el Consejo Nacional para el Desarrollo de los Recursos
Humanos en Salud (Colombia. Presidencia de la Rep?blica, 1992) y en la Ley 30
de 1992 (Colombia, Ley 30 de 1992, 1992).
La investigaci?n muestra c?mo, a lo largo de poco m?s de dos decadas, han
tenido lugar una serie de desplazamientos que atraviezan la educaci?n m?dica,
los cuales, soportados por mecanismos que orientados al logro de la meta antes
referida, tensionan las racionalidades pol?ticas que tienen que ver
particularmente con los sectores salud y educaci?n superior. Dichas tensiones
inciden en los saberes y en las estrategias del proceso de formaci?n,
contribuyendo a la configuraci?n de nuevas posiciones para los sujetos m?dicos
y constituyendo nuevos modos de funcionamiento de las relaciones m?dicopaciente,
en las que estudiantes de medicina, m?dicos y comunidad se ven
impelidos a autogestionarse.
Adoptar la perspectiva foucaultiana para adelantar el estudio implic?
precauciones epistemol?gicas relacionadas con la serie saber, poder, sujeto,
desde la cual se desarrollaron tres apartados del informe de tesis: el primero,
enfocado al saber, describe de acuerdo con la pertinencia, lo dicho por los
discursos que circulan sobre y en la educaci?n m?dica de pregrado, acerca de la
relaci?n m?dico-paciente, analizados en la modalidad de archivo. El segundo se
orienta al an?lisis del poder, en clave de tecnolog?as de gobierno que operan no
sobre los individuos, sino a partir de la regularizaci?n de los ambientes
educativos en que tiene lugar la conducci?n de la conducta de los estudiantes de
pregrado en su ir haciendose m?dicos generales. Por ?ltimo, la configuraci?n de
las posiciones de los sujetos, en un sentido que no se refiere a individuos
concientes que producen las pr?cticas discursivas, sino a sujetos producidos por
estas a partir de indicaciones y prescripciones que determinan sus modos de
hacer, hacerse y ser en el mundo.

Educaci?n m?dica, relaci?n m?dico paciente. This thesis problematizes medical education that is offered in undergraduate
programs of medicine in Colombia and its effects in the doctor-patient
relationship, which is assumed as a susceptible discourse practice to be studied,
making use of the tools proposed by Michael Foucault.
It started from a question because of the emergency of multiple declarative
transformations that the doctor-patient relationship has had in the last decades,
and the incidence of the government practices of medical education, in the
emergency of certain type of subject, that from their diverse position gives rise to
other emphasis and functioning forms of the doctor-patient relationship in
Colombia, in the period between 1992 and 2014.
The temporary delimitation places the start of the nineties as the time when
Colombia, in the frame of the Health and Equity Promotion Conference ? held in
Bogot? in 1992 (PAHO, WHO, 1992), adopts the recommendations suggested to
reach the goal ?Health for everyone in the year 200?, proposed during the
International Conference of Primary Health Care in Health, in Almaty, Kazakhstan
(WHO, 1978), and ratified in 1986 during the First International Conference for
Health Promotion, held in Ottawa, Canada (PAHO, 1986). Regarding medical
education, these recommendations were developed through the control and
management mechanism, established by the National Council for the
Development of Human Resources in Health (Colombia, Presidency of the
Republic, 1992) and in Law 30 of 1992 (Colombia, Law 30 of 1992, 1992).
Research shows how, along almost two decades, a series of displacements that
medical education passes has taken place, supported by mechanisms that
oriented towards the achievement of the previous goal, tension the political
rationalities that have to do mainly with Health sectors and Higher Education.

Such tensions impinge on the knowledge and the formation process strategies,
contributing to the setting of new positions for the medical parties y constituting
to the new forms of the doctor-patient relationship forms, in which medicine
students, doctors and the community are compelled to be self-policed.
To adopt the Foucauldian perspective to advance the study involved
epistemological cautions related to the series knowledge, power, subject, from
which three sections of the theses report were developed: the first one, focused
on knowledge, describes the agreement with the relevance, stated by the
discourses that circulate over and in the undergraduate medical education, about
the doctor-patient relationship, analyzed in the file mode. The second is oriented
to the analysis of power, in key government technologies which operate not over
the individuals, but from the regularization of educational environments in which
undergraduate students? behavior in their go takes place by becoming general
practitioners. Finally, the configuration of the positions of the subject, in a way that
does not refer to conscious individuals who produce discursive practices, but
subject produced by these from indications and prescriptions that determine their
ways of doing things, doing themselves and being in the world.
