Control de la hemostasia en pacientes diabéticos en cirugía de terceros molares incluidos

La diabetes mellitus provoca deterioro a nivel sistémico y bucal, causando estados protrombóticos producto de la alteración de plaquetas, fibrinólisis y coagulación sanguínea, podemos diferenciar diabetes tipo 1, 2 y gestacional, en las que existen factores predisponentes como edad, genética, raza, y hábitos alimenticios. En pacientes con obesidad e hiperglucemia se evidenció un aumento significativo de los factores VII, VIII, X y XII que participan en la cascada de coagulación así como del fibrinógeno inmunológico. La atención odontológica en estos pacientes debe ser completa y eficiente, controlar la hemostasia conlleva una serie de situaciones que van desde una correcta historia clínica, uso adecuado de anestésico y agentes hemostáticos y una técnica quirúrgica que evite lesionar tejidos circundantes o prolongar el proceso quirúrgico que limite el control de la hemorragia; el cual está comprendido por la intercomunicación entre el tejido y los factores participantes en el proceso de formación del tapón plaquetario así mismo el plasma sanguíneo y células del endotelio que en conjunto conforman el coágulo que tiene como función interrumpir el flujo de sangre. Los terceros molares incluidos repercuten considerablemente en la salud a nivel general y sistémico como infecciones, procesos cariosos y enfermedad periodontal, es más habitual localizarlos en la arcada inferior con notables distorsiones morfológicas en su parte coronaria como radicular, que en la mayoría de los casos cerca del 60% no hace contacto de oclusión y más del 50% no erupcionan fuera de la encía a lo cual se le denomina piezas dentarias retenidas o incluidas. The third molars included have a significant impact on general and systemic health such as infections, carious processes and periodontal disease, it is more common to locate them in the lower arch with notable morphological distortions in their coronary part such as root, that in most cases about 60% do not make occlusive contact and more than 50% do not erupt out of the gum which is called retained or included teeth.
