El tratamiento de los aneurismas intracraneales es uno de los grandes desafíos de la Neurocirugía y a pesar de los progresos su tratamiento es complicado. La cirugía endovascular se la define como alternativa mínimamente invasiva y en el Ecuador ocupa un lugar importante en el manejo de esta patología .La falta de evaluaciones y la no existencia de normas estandarizadas de la práctica inciden en la consideración de escoger el tratamiento más apropiado según indicaciones de seguridad y eficacia. Los objetivos de este trabajo es analizar y evaluar la evolución postoperatoria de los pacientes tratados mediante embolización, con el fin de determinar el efecto de dicha terapéutica, así como elaborar un protocolo para mejorar su indicación. El presente es un estudio tipo descriptivo, correlacional, diseño no experimental, longitudinal retrospectivo realizado en el Hospital Regional 2 del IESS “Teodoro Maldonado Carbo”, en base a una muestra de 42 pacientes con diagnóstico de aneurisma cerebral que fueron intervenidos utilizando técnicas endovasculares. Los resultados se analizaron con medidas estadísticas descriptivas en la que se establecieron los efectos de la técnica, basados en los datos obtenidos de las historias clínicas. Dado el amplio uso del tratamiento endovascular, es importante establecer recomendaciones, basados en la mejor evidencia disponible, para definir indicaciones apropiadas para este tratamiento en el contexto de alternativas quirúrgicas. Estas consideraciones ayudarían al desarrollo de programas de mejoramiento y de valoración del procedimiento, que se traduce al final en grandes contribuciones a la calidad de atención de los pacientes. Los resultados del estudio sugieren que el tratamiento endovascular demuestra rangos bajos de morbimortalidad en paciente apropiadamente seleccionados, lo que demuestra los beneficios de la terapia endovascular y alienta su aplicación con indicaciones precisas y su valoración constante para su mejoramiento The treatment of the aneurysms intracranial is one of the big challenges of the Neurosurgery and in spite of the progresses its treatment is complicated. The surgery endovascular is defined it minimumly as alternative invasiva and in the Ecuador it occupies an important place in the handling of this pathology. The lack of evaluations and the non existence of standardized norms of the practice impact in the consideration of choosing the most appropriate treatment according to indications of security and effectiveness. The objectives of this work are to analyze and to evaluate the postoperative evolution of the patient’s treaties by means of embolization, with the purpose of determining the effect of this therapy, as well as to elaborate a protocol to improve their indication. The present is not a study descriptive type, correlacional, design experimental, longitudinal retrospective carried out in the Regional Hospital 2 of the IESS "Teodoro Maldonado Carbo", based on a sample of 42 patients with diagnostic of cerebral aneurysm that were intervened using technical endovascular. The results were analyzed with descriptive statistical measures in which the effects of the technique settled down, based on the obtained data of the clinical histories. Given the wide use of the treatment endovascular, is important to establish recommendations, based on the best available evidence, to define appropriate indications for this treatment in the context of alternative surgical. These considerations would help to the development of programs of improvement and of valuation of the procedure that he/she translates himself at the end in big taxes to the quality of the patients' attention. The results of the study suggest that the treatment endovascular demonstrates low ranges of morbimortality appropriately in patient selected, what demonstrates the benefits of the therapy endovascular and it encourages its application with precise indications and its constant valuation for its improvement
