Introduction: neuromuscular relaxation is fundamental in surgical practice and is used in anesthesiology, resuscitation and treatment of critically ill patients. Objective: to compare the time of action of rocuronium in patients with overweight or obesity and normal weight through the TOF test, in the hospitals José Carrasco Arteaga and Vicente Corral Moscoso. Material and methods: cohort study that compared the time of onset of the action of the relaxant, the time of onset of the action, the recovery time of 90% of TOF or TOF of 0.9 in patients with normal weight and patients with overweight u Obesity, 246 patients met. The inclusion criteria. For the cohort exposed to the risk factor (overweight and obesity) were 123 patients and for the non-exposed cohort (normal weight) 123 patients with a ratio of 1: 1 patients. For the hypothesis test the values of CI, 95% CI and p value were used, one p <0.05 was accepted. Results: The onset of action of the drug was higher in overweight and obese patients. In obese and overweight patients, muscle blockage increases almost six times more RR 5.75 (2.04-16.13) p 0.00; Neuromuscular recovery was greater in overweight and obese patients (p..014); there was no statistical difference in the need for reversion of muscle relaxant RR 1.12 (0.83-1.50). Conclusions: patients with overweight and obesity have a higher risk of late onset of action, neuromuscular block and neuromuscular recovery than patients with normal weight.
