Dexmedetomidina : aliado o enemigo en la intubación del paciente despierto? una revisión sistemática

Introduction: as of today there is no a drug or a mixture to intubate the patient awake, there are many medications that have been used trying to get the right conditions of sedation, analgesia, enabling the cooperation of the patient to be an effective procedure and not unpleasant. Dexmedetomidine is an emerging drug that provides the right conditions of sedation for awake intubation. This review evaluates the use of dexmedetomidine sedation for awake intubation through a systematic literature review.
Methods: We searched electronic databases in different publications from 1997 to today, for a total of 29 articles that were analyzed.
Results: Dexmedetomidine provides adequate sedation, amnesia, analgesia, anxiolysis and decreased production of secretions in the airway, without significant hemodynamic awake intubation.
Discussion: Dexmedetomidine sedation is useful for procedures that require the patient is comfortable but to obey orders to facilitate the work of operador. Showed utility in sedation for intubation of the awake patient, as in the case of patients with risk of neurologic injury during intubation or positioning for surgery, in patients with anterior mediastinal masses by the advantage of preserving spontaneous ventilation as a protective measure against the collapse of the airway
