Predicción de falla renal: Validación externa multicéntrica del modelo KFRE en pacientes de EsSalud con ERC en estadíos 3-4. Reporte de Resultados de Investigación 01-2023

Objetivo: Validar externamente el modelo de predicción multivariable KFRE (Kidney Failure Risk Equation) de 4 variables para predecir el riesgo de falla renal a 2 y 5 años en pacientes con enfermedad renal crónica (ERC) estadios 3-4 de EsSalud. Metodología: Cohorte retrospectiva de pacientes mayores de 18 años, diagnosticados con ERC estadio 3-4, entre el 01 de enero 2013 y el 31 de diciembre de 2017 en todos los establecimientos de salud de la Red Asistencial Rebagliati; que tuvieran datos disponibles de las variables del modelo KFRE. El desenlace fue la falla renal. Los pacientes fueron seguidos hasta que desarrollaron falla renal, fallecieron sin ésta, se perdieron o terminaron el estudio (31 de diciembre de 2019), lo que ocurrió primero. Evaluamos el rendimiento predictivo de los modelos KFRE mediante medidas de discriminación y calibración teniendo en cuenta a la muerte sin diálisis como riesgo competitivo. Resultados: 7519 pacientes en estadios 3-4, de 17 establecimientos de EsSalud fueron evaluados. La incidencia acumulada estimada de falla renal a 2 años y a 5 años fue de 1,52% (1,24% a 1,79%) y 3.37% (2,95% a 3,8%), respectivamente. La discriminación de KFRE a 2 y 5 años fue alta, con C/D AUC-td e índices C > 0,8 para todas las poblaciones. Respecto a métricas de calibración, la razón O/E y el intercepto de calibración indican que KFRE a 2 años subestima el riesgo, mientras que lo sobreestima a 5 años. Conclusión: Los modelos KFRE de cuatro variables tienen buena discriminación; respecto a la calibración subestima el riesgo de falla renal a corto plazo y lo sobreestima a largo plazo. KFRE debería ser recalibrado para nuestra población para poder usarse en las guías de práctica clínica locales.
