Umbral de glucemia predictivo en el pronóstico evolutivo del ictus

Identificar el umbral de glucemia plasmática a partir del cual se asocia una peor evolución en los pacientes con ictus atendidos en el Hospital Regional Docente de Trujillo y Hospital Belén de Trujillo durante el periodo Noviembre 2015-Abril 2016.
Materiales y Método: Estudio multicéntrico, prospectivo y observacional que incluye
pacientes con ictus de menos de 24h de evolución. Se registró la glucemia capilar (en
emergencias y dos veces al día en las primeras 48h). Se consideró mal pronóstico a una
puntuación en la Escala de Rankin modificada >2 a los tres meses. Resultados: Se
incluyó a 129 pacientes. La edad media de los pacientes fue de 70,26 ± 13,77 años de
edad y el 51,2% (66) fue de sexo masculino. El 72,9% (94) del total de muestra
correspondieron a ictus isquémico y el 27,1 % (35) a hemorrágico. El análisis de curvas
ROC para la glicemia en emergencia mostró un ABC de 0,822 (95% IC 0,746-0,899) y
para la glicemia máxima en las primeras 48 h un ABC de 0,808 (95% IC 0,729-0,886);
identificándose los puntos de 136 mg/dl para emergencia (sensibilidad 67,3%; especificidad 93,2%) y de 147 mg/dl de glicemia máxima en las primeras 48 h
(sensibilidad 61,8 %; 90% especificidad) como los puntos de corte que predicen muerte
o dependencia a los 3 meses respectivamente. El 32,6% de los pacientes tenían una
glicemia en emergencia mayor o igual a 147 mg/dl y un 52,7% de los pacientes
presentaron una glicemia en las primeras 48 h mayor o igual a 147 mg/dl. Conclusión:
El umbral de glicemia plasmática ≥ 147 mg/dl en las primeras 48 h es un factor
predictivo de un mayor riesgo de muerte y dependencia a los tres meses tras un ictus
