Disminución del índice de choque como valor pronóstico de sobrevivencia en pacientes con choque séptico pediátrico. Hospital Regional Docente de Trujillo, período 2008-2013

Determinar si la disminución del índice de choque (IC) tiene valor pronóstico de sobrevivencia en pacientes con choque séptico pediátrico. Materiales y métodos: Estudio observacional, longitudinal de tipo cohorte retrospectiva realizado en el Hospital Regional Docente de Trujillo (HRDT). Se revisaron 36 historias clínicas de pacientes mayores de 28 días y menores de 15 años con diagnóstico de choque séptico, hospitalizados en la Unidad de Cuidados Especiales Pediátricos del Departamento de Pediatría del HRDT, entre Enero del 2008 y Diciembre del 2013, en los cuales se evaluó la disminución del ÍC a las 24 horas respecto a su valor inicial y su sobrevivencia. Se consideró como disminución del IC a una reducción mayor o igual al 22% respecto al valor inicial. Resultados: De 22 pacientes sin disminución del IC, 6 pacientes sobrevivieron (27.3%) y 16 no sobrevivieron (72.7%), y de los 14 pacientes con disminución del IC, 12 pacientes sobrevivieron (85.7%) y 2 no sobrevivieron (14.3%). La probabilidad de sobrevivencia hallada fue de 3.14, el de Chi cuadrado (χ²): 11.6883, con intervalo de confianza al 95% de 1,54 - 6,43; y un valor de p = 0,0006. Conclusiones: La disminución del Índice de choque si tiene valor pronóstico de sobrevivencia en pacientes con choque séptico pediátrico
