Prevalencia De Anemia Y Su Relación Con La Fracción De Eyección Y La Clase Funcional En Pacientes Adultos Con Insuficiencia Cardiaca Crónica.

Fundamento: La insuficiencia cardiaca (IC) es considerada como un problema de salud pública en rápido crecimiento y su relación con la anemia es un hecho que ha tomado una reciente importancia, pero es poco considerada en la práctica clínica. Los estudios que midieron la prevalencia de anemia en los pacientes con IC tienen resultados muy variables por lo que la magnitud del problema no está del todo clara. Algunos trabajos han estudiado la relación entre la anemia y la severidad de la IC, días de hospitalización y la mortalidad y han mostrado que existe una relación significativa entre la anemia y estas variables. Objetivo: Determinar la prevalencia de anemia y su relación con la clase funcional y la fracción de eyección en los pacientes adultos con Insuficiencia Cardiaca Crónica de la Clínica San Pablo durante el periodo 2002-2013. Método: Se escogieron 176 historias clínicas de manera aleatoria de pacientes con diagnóstico de IC en la Clínica San Pablo durante el periodo 2002-2013. Se recogieron los datos de edad, género, nivel de hemoglobina, fracción de eyección y clase funcional NYHA (New York Heart Association). Se halló la prevalencia de anemia y el promedio de hemoglobina y se determinó la relación con la fracción de eyección y la clase funcional NYHA. Resultados: De la muestra de 176 historias clínicas, se encontró que 102 pacientes (57.95%) fueron varones y 74 pacientes (42.05%) fueron mujeres, con una edad promedio de 75.37 años y un nivel de hemoglobina de 11.98 g/dl. Mediante la prueba de chi-cuadrado se determinó que si existe una relación altamente significativa entre la prevalencia de anemia y la fracción de eyección y la clase funcional NYHA (p<0.001). También se observó que la media de hemoglobina fue cayendo progresivamente desde 13.48 g/dl en la clase funcional NYHA I hasta 10.63 g/dl en la clase funcional NYHA IV (p<0.001) y desde 12.33 g/dl en el grupo con FE normal hasta 10.81 g/dl en el grupo con FE disminuida (p <0.05). Conclusión: Si existe relación entre la prevalencia de anemia y la fracción de eyección y la clase funcional. Los niveles de hemoglobina son menores en cuanto más severa se la IC y más afectada esté la función sistólica.
