Un Problema Relacionado con Medicamento (PRM) es cualquier evento de salud presente en un paciente, producido o relacionado con su tratamiento fármaco terapéutico y que interfiere con los resultados de salud esperados. Es evidente la alta incidencia de polifarmacia en la tercera edad y el riesgo de presentar PRM.Se realizó un estudio descriptivo, transversal y retrospectivo en adultos mayores ingresados en el Hospital Nacional Guillermo Almenara Irigoyen con polimedicación, en un período comprendido entre Mayo 2011-Noviembre 2011 con el objetivo de determinar la incidencia de PRM en esta población.Se estudiaron 60 pacientes, encontrándose un total de 53 PRM. La mayor frecuencia (37.74 %) incluyó problemas de seguridad, debido fundamentalmente a: Interacciones Medicamentosas Potenciales (IMP) y Reacciones Adversas a Medicamentos (RAM), evidenciándose la necesidad del análisis cuidadoso de los esquemas de tratamientos impuestos, con el fin de minimizar los riesgos que la presencia de PRM representa para los mismos
