Factores de riesgo asociados al Desgarro Perineal en partos atendidos en Centros Maternos Infantiles de Chorrillos durante el periodo del 2021 al 2022

Introducción: El desgarro perineal es la complicación obstétrica más frecuente
en los partos por vía vaginal atendidos en el primer nivel de atención. El 91% de
madres reporta signos y síntomas hasta 8 semanas postparto de dolor perineal,
incontinencia urinaria y/o fecal, disfunción sexual, dispareunia, fístulas
rectovaginales, infecciones del tracto urinario a repetición y prolapsos genitales,
siendo estos tres últimos las complicaciones más graves. Todas estas
complicaciones ocasionan un impacto negativo en la salud física y mental de las
madres.
Objetivo: Determinar los factores de riesgo asociados al desgarro perineal en
partos atendidos en Centros Maternos Infantiles de Chorrillos durante el periodo
del 2021 al 2022.
Métodos: Estudio observacional, analítico, longitudinal, retrospectivo, casos y
controles. Se estudiaron 990 puérperas, divididas en grupos de 330 casos y 660
controles, con una frecuencia de exposición entre los controles de 78.5%, odds
ratio previsto de 1.68, nivel de confianza del 95%, un poder estadístico de 83.5%
y la proporción de casos y controles de 2:1. No se aplicó técnicas de muestreo
debido a que se utilizó el total de la población. El análisis considerado fue
significativo con un p < 0.05.
Resultados: Los resultados obtenidos fueron ajustados mediante un modelo de
regresión logística binaria. En el análisis multivariado, se identificó como factor
de riesgo a la edad materna adolescente (ORa 60,76, IC95% 7,56-
488,10, p < 0,001), la nuliparidad (ORa 17,65, IC95% 3,33-93,30, p < 0,05),
número inadecuado de controles prenatales (ORa 1,75, IC95% 1,07-2,87, p
< 0,05), ausencia de episiotomía (ORa 57,57, IC95% 35,57-93,20,
p valor < 0,001); por otro lado, se identificaron como factor protector a la edad
adulta avanzada (ORa = 0,02, IC95%: 0,01-0,06, p < 0,001) y el peso fetal
normosómico (ORa 0,11, IC95% 0,03-0,43, p< 0,05).
Conclusiones: En nuestra investigación, las variables de riesgo con un grado
de asociación estadísticamente significativo fueron el ser adolescente, el ser
nulípara, el tener un inadecuado número de controles prenatales y la falta de
episiotomía, no se evidenciaron factores de riesgo neonatales. Con respecto a
las variables protectoras, el ser madre con edad avanzada y el tener un recién
nacido con peso normosómico tuvieron significancia estadística
