Viral hepatitis is currently a relevant public health issue in Brazil and the world; it is
ubiquitous, reaching several segments of the population and causing severe morbidity and
mortality. The standard treatment for viral hepatitis C during the last few decades has been
combination therapy with alpha-pegylated interferon (PEG-IFN) and ribavirin (RBV), a
guanosine analog that disrupts viral RNA metabolism. The treatment of chronic forms is
based on this combination of drugs, for a period of 24–48 weeks, depending on the viral
genotype found. Hepatitis C has a high impact on global public health. Recent studies suggest
the involvement of oxidative stress in the pathogenesis of hepatitis C and hepatocellular
carcinoma. Oxidative cellular damage plays an important role in the pathophysiology of
hepatitis C virus (HCV). Oxidative stress is triggered when the concentration of oxygen
species in the extracellular or intracellular environment exceeds the capacity for antioxidant
defense. The exact mechanism by which this occurs is not fully understood, and immune
hepatic damage, direct cytotoxic damage mediated by different viral products, and oxidative
stress have been implicated in the pathogenesis of hepatitis C. . In recent years, the
involvement of oxidative stress in the progression of hepatitis C has been demonstrated, and
the immune response increases the production of reactive oxygen species. However, little is
known about the involvement of oxidative stress in hepatitis C patients on combined
treatment with PEG-IFN and RBV. Thus, the objective of this study was to determine
oxidative stress profile in hepatitis C patients who used combination therapy with pegylated
interferon alfa and ribavirin. A total of 43 patients with viral hepatitis C were recruited at the
referral service in the city of Uruguaiana, and venous blood was collected. The HCV group
was divided into four groups: diagnosis (free of combined pharmacological treatment), time 1
(onset of combined treatment), time 2 (half of the combined treatment), and time 3 (final of
the combined treatment). The parameters of oxidative stress were determined by classical
methodologies. The results show that HCV carriers had a significant increase in markers of
hepatic function, and the activities of GOT (glutamic-oxaloacetic transaminase) and TGP
(glutamic-pyruvic transaminase) are reestablished after the combined treatment. Enzymatic
and non-enzymatic defenses were decreased in the HCV group relative to the control group,
and combined antiviral therapy sometimes reversed these parameters. Oxidative damage to
biomolecules was already increased in these carriers, and combined treatment improved these
parameters, but in the control group, levels remained unchanged. Thus, the results suggest that HCV carriers have increased oxidative stress at the time of diagnosis, and combined treatment seems to improve this condition. As hepatites virais constituem atualmente uma relevante questão de saúde pública no Brasil e
no mundo, distribuindo-se de maneira universal, atingindo vários segmentos da população e
causando grande impacto de morbidade e mortalidade. O tratamento padrão para a hepatite
viral C durante as últimas décadas foi a terapia combinada de interferon alfa-peguilado (PEG-
IFN) com ribavirina (RBV), um análogo de guanosina que interrompe o metabolismo de RNA
viral. O tratamento das formas crônicas baseia-se nesta combinação de fármacos, por um
período de 24 a 48 semanas, dependendo do genótipo viral encontrado. A hepatite C apresenta elevado impacto na saúde pública global. Estudos recentes sugerem o envolvimento do estresse oxidativo na patogenia da hepatite C e carcinoma hepatocelular (CHC). O dano
celular oxidativo desempenha um papel importante na fisiopatologia do vírus da hepatite C
(HCV). O estresse oxidativo é desencadeado quando a concentração de espécies de oxigênio
no ambiente extracelular ou intracelular excede as defesas antioxidantes. O mecanismo exato
pelo qual isto ocorre não está totalmente compreendido, incluindo dano hepático imunológico, dano diretamente citotóxico mediado por diferentes produtos virais, bem como o estresse oxidativo, têm sido sugeridos na patogênese da hepatite C. Nos últimos anos tem sido demonstrado o envolvimento do estresse oxidativo na progressão da hepatite C, sendo que a resposta imune aumenta a produção de espécies reativas de oxigênio (ERO). Entretanto,
pouco se sabe sobre o envolvimento do estresse oxidativo nos portadores da hepatite C em
tratamento combinado com PEG-IFN e RBV. Assim, o objetivo deste trabalho foi avaliar o
perfil de estresse oxidativo em portadores da hepatite C que fizeram uso da terapia combinada
com interferon alfa peguilado e ribavirina. Foram recrutados 43 portadores de hepatite viral C
no serviço de referência do município de Uruguaiana e submetidos a uma coleta de sangue
venoso. Foram divididos em quatro grupos: diagnosticado com HCV (livre de tratamento
farmacológico combinado), tempo 1 (início do tratamento combinado), tempo 2 (metade do
tratamento combinado) e tempo 3 (final do tratamento combinado). Os parâmetros de estresse oxidativo foram determinados por metodologias clássicas. Os resultados mostram que os portadores de HCV possuem aumento significativo dos marcadores de função hepática, sendo que as atividades da TGO (transaminase glutâmico-oxalacética) e TGP (transaminase
glutâmico-pirúvica) são reestabelecidas após o tratamento combinado. Foi observado também
que as defesas enzimáticas e não enzimáticas estão diminuídas no grupo HCV em comparação
ao grupo controle e a terapia antiviral combinada por vezes, consegue reverter esses
parâmetros. Já o dano oxidativo a biomoléculas está aumentado nesses portadores e o
tratamento combinado melhora esses parâmetros, porém não em níveis do grupo controle.
Assim, os resultados sugerem que o portador de HCV tem aumento do estresse oxidativo no
momento do diagnóstico e o tratamento combinado parece melhorar essa condição.
