Avaliação dos tipos de tratamentos farmacológicos e não farmacológicos para Síndrome do Ovário Policístico

A Síndrome dos Ovários Policísticos (SOP) é uma das desordens endócrinas metabólicas mais frequentes em mulheres na idade reprodutiva, acometendo em torno de 9% a 18% da população feminina em idade fértil. A condição é heterogênea, e as mulheres podem apresentar uma série de complicações reprodutivas (disfunção menstrual, infertilidade e complicações na gravidez), endócrinas (hiperandrogenismo, hirsutismo e acne), metabólicas (resistência à insulina, diabetes, ganho de peso e obesidade) e sintomas psicossociais (ansiedade, depressão e má qualidade de vida). Ante o exposto, o presente trabalho tem com objetivo realizar uma síntese da literatura dos estudos sobre tratamentos farmacológicos e não farmacológicos que impactam positivamente no tratamento e sintomatologia da SOP. Na busca de artigos disponíveis na literatura científica foram identificados até abril de 2014 usando as bases de dados PubMed/MedLine; Cochrane library; Scieloe Lilacs. Os dois revisores realizaram a seleção e analisarm os títulos, resumo e texto completo, identificando os critérios de elegibilidade. As divergências de seleção foram resolvidas através de um terceiro revisor. No que diz respeito à linha temporal dos artigos avaliados 30,76% (n=4) equivale ao ano 2018. Quanto aos estudos, foi observado às medidas não farmacológicas e farmacológicas em que 62% (n=8) tratam-se de intervenções relacionadas a mudanças na dieta e estilo de vida saudável e, uso de fitoterápicos pelas pacientes. Visto que, 62% dos estudos se relacionam a medidas não farmacológicas e 38% medidas farmacológicas. O uso de pílula anticoncepcional oral combinada (COCP) e metformina nas condições hormonais e clínicas da SOP como medidas farmacológicas podem proporcionar benefícios para às características metabólicas da síndrome. A observação dos desfechos endócrinos, metabólicos e antropométricos das pacientes com SOPcontribuem para o manejo das manifestações clínicas, bioquímicas e metabólicas apresentadas. Já em relação às medidas não farmacológicas, às intervenções no estilo de vida, uso de suplementos nutricionais e a realização de terapias medicinais alternativas combinadas ou não, com metformina são estudadas em relevância na literatura, como tratamentos de primeira linha para mulheres com SOP. Diante disso, pode-se constatar que houve melhores evidências clínicas no uso de medidas farmacológicas associadas a práticas alternativas em pacientes portadores de SOP. Contudo, a necessidade de ensaios clínicos randomizados com metodologia robusta para garantir a qualidade e
segurança dos estudos. PCOS syndrome is one of the most frequent metabolic, endocrine disorders in women of reproductive
age, affecting around 9% to 18% of the female population of childbearing age. The condition is
heterogeneous, and women may present with several reproductive complications (menstrual
dysfunction, infertility, and complications in pregnancy), endocrine (hyperandrogenism, hirsutism and
acne), metabolic (insulin resistance, diabetes, weight gain and obesity) and psychosocial symptoms
(anxiety, depression, and poor quality of life). In the light of the above, the present study aims to
synthesis a synthesis of the literature of studies on pharmacological and non-pharmacological
treatments that positively impact the treatment and symptomatology of PCOS. In the search for
articles available in the scientific literature were identified until April 2014 using the PubMed/MedLine
databases; Cochrane Library; Scielo and Lilacs. The two reviewers made the selection and analyzed
the titles, abstract and full text, identifying the eligibility criteria. The selection divergences were
resolved through a third reviewer. Regarding the timeline of the articles evaluated, 30.76% (n=4) is
equivalent to the year 2018. Regarding the studies, it was observed the non-pharmacological and
pharmacological measures in which 46% (n=9) are interventions related to changes in diet and healthy
lifestyle and the use of herbal medicines by patients. Since, 62% of the studies are related to
pharmacological measures and 38% to non-pharmacological measures. The use of combined oral
contraceptive pill (COCP) and metformin in hormonal and clinical conditions of PCOS as
pharmacological measures may provide benefits to the metabolic characteristics of the syndrome. The
observation of endocrine, metabolic, and anthropometric outcomes of patients with PCOS contribute
to the management of the clinical, biochemical and metabolic manifestations presented. In relation to
non-pharmacological measures, lifestyle interventions, use of nutritional supplements and the
performance of alternative medicinal therapies combined or not, metformin are studied for relevance in
the literature, as first-line treatments for women with PCOS. Therefore, there was better clinical
evidence in the use of pharmacological measures associated with alternative practices in patients with
PCOS. However, the need for randomized clinical trials with robust methodology to ensure the quality
and safety of studies.
