Caracterização clínica e genética de pacientes com transtorno do espectro autista atendidos em um ambulatório universitário no Sul do Brasil

INTRODUCTION: Autism spectrum disorder (ASD) is an evolutionary-behavioral disorder, which has increased in prevalence in recent years, with current rates of 1:54 children. Genetic predisposition and several environmental factors are being implicated as causes of autism, but the precise pathophysiological mechanisms remain unknown. In this context, the term syndromic autism is used to refer to cases of ASD that have a defined cause. A complementary investigation method for the etiological determination of ASD is neurogenetic investigation, and autism can have a hereditary behavior in up to 90% of cases. The purpose of this article was to evaluate the occurrence of syndromic autism and the genetic characteristics involved in patients with ASD. METHODS: an observational, cross-sectional and retrospective study was carried out using data collected from the medical records of 135 children, aged between 2 and 15 at the time of the consultation, who underwent genetic investigation and were diagnosed with ASD by the UNISUL Maternal and Child Ambulatory in Tubarão, Santa Catarina Brazil. RESULTS: According to the results of the genetic tests, 20% of the patients who underwent some type of genetic investigation changed and were included in the group of atypical autists. Among the patients who underwent the CGH Array exam 70.97% had some genetic alteration found, among those who underwent the karyotype 5.26% had changes, followed by 2.31% of the changes in the patients who underwent Fragile X DNA test. When the study variables were compared, comparing the groups with normal and abnormal genetic tests, it was found that the presence of dysmorphism and microcephaly had statistical significance in the probability of alteration in the investigation. CONCLUSIONS: In the etiological assessment of autism spectrum disorder, in addition to clinical history and detailed physical examination, it is important to carry out the genetic test, which should be offered to all families. The karyotype, Fragile X DNA test and CGH Array detected the etiology in 20% of the children studied. In patients with some dysmorphism, the tests appear to be more sensitive. INTRODUÇÃO: O transtorno do espectro autista (TEA) é um transtorno evolutivo-comportamental, que apresentou aumento da sua prevalência nos últimos anos, com atuais índices de 1:54 crianças. A predisposição genética e diversos fatores ambientais estão sendo implicados como causas do autismo, porém os mecanismos fisiopatológicos precisos permanecem desconhecidos. Nesse contexto, o termo autismo sindrômico é utilizado para se referir aos casos de TEA que possuem uma causa definida. Um método de investigação complementar para a determinação etiológica do TEA é a investigação neurogenética, sendo que o autismo pode ter comportamento hereditário em até 90% dos casos. Esse artigo teve como objetivo avaliar a ocorrência de autismo sindrômico e as características genéticas envolvidas em pacientes com TEA. MÉTODOS: Estudo observacional, transversal e retrospectivo foi realizado a partir de dados coletados no prontuário de 135 crianças, que possuíam entre 2 e 15 anos no momento da consulta, que realizaram investigação genética e que foram diagnosticadas com TEA pelo Ambulatório Materno Infantil da UNISUL em Tubarão, Santa Catarina, Brasil. RESULTADOS: De acordo com os resultados dos exames genéticos, 20,00% dos pacientes que realizaram algum tipo de investigação genética tiveram alteração e foram incluídos no grupo de autistas atípicos. Entre os pacientes que realizaram o exame de CGH Array 70,97% tiveram alguma alteração genética encontrada, entre os que realizaram cariótipo 5,26% tiveram alterações, seguido de 2,31% de alterações nos pacientes que fizeram o PCR para X frágil. Quando cruzadas as variáveis do estudo comparando os grupos com exames genéticos normais e anormais, verificou-se que a presença de dismorfismo e a microcefalia tiveram significância estatística na probabilidade de alteração na investigação genética. CONCLUSÕES: Na avaliação etiológica do transtorno do espectro autista, além da história clínica e do exame físico detalhado é importante a realização do teste genético, que deveria ser oferecido a todas as famílias. O cariótipo, o PCR para X fragil e o CGH Array detectaram a etiologia em 20,00% das crianças estudadas. Em pacientes com algum dismorfismo os testes parecem ser mais sensíveis.
