Custo-utilidade do tratamento primário não farmacológico do glaucoma primário de ângulo aberto no sistema público de saúde brasileiro

Glaucoma is a genetic and chronic disease, defined as a typical optic neuropathy that
results in functional and structural damage. Primary open-angle glaucoma is the most
prevalent type of glaucoma, and treatments aim at controlling intraocular pressure,
either by surgery, medication or laser. There are factors that can cause failure in clinical
treatment, such as drugs´ excessive cost and side effects. As a consequence of the
increase in life expectancy of the Brazilian population, the incidence of glaucoma tends
to rise significantly, generating an increase in its economic impact on the expenses of
patients, health systems and society as a whole. The objective of the study is to
evaluate the economic impact of initiating the treatment of POAG with nonpharmacological strategies within the reference centers for the treatment of glaucoma
by the SUS. Three treatment strategies were studied: the first follows the usual SUS
therapy; strategies 2 and 3 have the same treatment initiation, using SLT laser, but
what differs them is the therapeutic option after SLT. The Markov model was chosen
based on the characteristics of the pathology under study, as it is a chronic disease
with recurrent and growing costs, using the incremental cost-utility ratio (ICUR) as an
outcome measure, in which the incremental cost per benefit is achieved (R$/QALY).
Strategies 2 and 3 generated a gain in quality of life, and were dominant over strategy
1, being more effective and cheaper. However, the cost-effectiveness of strategy 2
suffers a significant impact according to the age of entry into the model. Therefore, we
conclude that when starting the treatment of initial POAG with SLT, there is a gain in
quality of life and a reduction in future costs in relation to the usual treatment with eye
drops. The use of the trabecular device after laser failure generates an even greater
gain in quality of life, and its cost, although higher, can be considered cost-effective
from the perspective of SUS. Glaucoma é uma doença genética e crônica, definida como uma neuropatia óptica
típica que resulta em um dano funcional e estrutural. O glaucoma primário de ângulo
aberto é o mais prevalente dentre os tipos de glaucoma, e os tratamentos visam
controlar a pressão intraocular, seja através de cirurgia, medicamentos ou laser.
Existem alguns fatores que podem causar insucesso no tratamento clínico, como
custo elevado dos medicamentos e seus efeitos colaterais. Como consequência do
aumento da expectativa de vida da população brasileira, a incidência de glaucoma
tende a se elevar significativamente, gerando aumento de seu impacto econômico
sobre os gastos dos pacientes, dos sistemas de saúde e da sociedade como um todo.
O objetivo do estudo é avaliar o impacto econômico de se iniciar o tratamento do
GPAA com estratégias não farmacológicas no âmbito dos centros de referência para
o tratamento do glaucoma pelo SUS. Foram estudadas 3 estratégias de tratamento: a
primeira segue a terapia usual do SUS; as estratégias 2 e 3 possuem o mesmo início
de tratamento, utilizando laser SLT, mas o que as difere é a opção terapêutica após o
SLT. Optou-se pelo modelo de Markov com base nas características da patologia em
estudo, por se tratar de uma doença crônica com custos recorrentes e crescentes,
com a utilização da razão de custo-utilidade incremental (ICUR) como medida de
desfecho, na qual se evidencia o custo incremental por benefício atingido (R$/QALY).
As estratégias 2 e 3 geraram um ganho de qualidade de vida e foram dominantes
sobre a estratégia 1, sendo mais efetivas e mais baratas. Porém, a relação custoefetividade da estratégia 2 sofre um impacto significativo de acordo com a idade de
entrada no modelo. Conclui-se, portanto, que, ao se iniciar o tratamento do GPAA
inicial com SLT, há um ganho de qualidade de vida e uma redução nos custos futuros
em relação ao tratamento habitual com colírios. O uso do dispositivo trabecular após
a falha do laser gera um ganho ainda maior de qualidade de vida, e seu custo, apesar
de maior, pode ser considerado custo-efetivo sob a perspectiva do SUS.
