Influência da plataforma protética na distribuição de tensões em implantes extracurtos: in silico e in vitro

The aim of the study was to evaluate through Finite Elements (FEA) and Extensometer the
biochemical benefit of the prosthetic connections as for the distribution of tensions for the perimplant
bone cloth, implants and components, when we use extra short implants in subsequent
regions of atrophic jaw. For evaluation for extensiometry twenty bodies of proof were made
and divided in two groups: Group Splinted Morse Taper (SMT) and Splinted External Hexagon
(SEH), each body constituted by two extra short implants of 5 x 4 mm concentrated on a block
of polyurethane. On the implants prosthetic crowns were installed when crown / implant of 2:1
screwed relative to the first and second thing molars with a proportion. The bodies of proof
were subjected to a load of de 120N in a machine of mechanical fatigue, when there registered
the values of the deformation suffered in the model of polyurethane through extensometers
fixed on basis of the polyurethane. For evaluation by FEA a three-dimensional model was raised
simulating virtually the same situation, being the values of bone deformation cortical and to run
through registered. A less deformation was observed in the FEA in the group SMT, 0.0095μs
for the bone cortical and 0.028 μs for the medullar bone, and in the extensometry the group
SMT presented less deformations in the bone cortical, in the regions distal, lingual and college
entrance exam of the corresponding implant to the first molar (p 0,016, 0,028 and 0,028
respectively) .With that we end that the platform Morse Taper favored a less bone marginal
deformation and a better uniformity in distribution of tension for the prosthetic component
when we use extra short implants. O objetivo do estudo foi avaliar por meio de Elementos Finitos (FEA) e Extensometria o
benefício biomecânico das conexões protéticas quanto à distribuição de tensões para o tecido
ósseo periimplantar, implantes e componentes, quando utilizamos implantes extracurtos em
regiões posteriores de mandíbula atrófica. Para avaliação por extensiometria foram
confeccionados vinte corpos de prova e divididos em dois grupos: Grupo Cone Morse
Esplintado (CMES) e Hexágono externo Esplintado (HEES), cada corpo constituído por dois
implantes extracurtos de 5 x 4 mm fixados em um bloco de poliuretano. Sobre os implantes
foram instaladas coroas protéticas parafusadas relativas ao primeiro e segundo molares com
uma proporção coroa/implante de 2:1. Os corpos de prova foram submetidos a um carregamento
de 120 N em uma máquina de fadiga mecânica, sendo registrados os valores da deformação
sofrida no modelo de poliuretano através de extensômetros fixados na base do poliuretano. Para
avaliação por FEA um modelo tridimensional foi criado simulando virtualmente a mesma
situação, sendo os valores de deformação óssea cortical e medular registrados. Foi observado
no FEA uma menor deformação no grupo CMES, 0.0095μs para o osso cortical e 0.028 μs para
o osso medular, e na extensometria o grupo CMES apresentou menores deformações no osso
cortical, nas regiões distal, lingual e vestibular do implante correspondente ao primeiro molar
(p 0,016, 0,028 e 0,028 respectivamente). Com isso concluímos que a plataforma Cone Morse
favoreceu uma menor deformação óssea marginal e uma melhor uniformidade em distribuição
