Introduction: Respiratory diseases affect millions of people worldwide, being the most frequent causes of illness and deaths reaching all age groups, mainly children and the elderly. In Brazil, hospital admission rates due to respiratory diseases (ICD-10X) occupy the first place in the country, with great socioeconomic impact. Objective: To evaluate the muscular strength and functionality of patients hospitalized for acute and chronic acute respiratory diseases. Method: A cross-sectional study performed in patients diagnosed and hospitalized with acute or chronic acute respiratory diseases. The evaluations were performed through the Medical Research Council (MRC), Functional independence measure (MIF), manovacuometry and palmar grip dynamometry in patients admitted to the Lagarto University Hospital. Results: Fifty-six patients were included in the analysis, 40 belonging to the G1 group (acute respiratory disease) and 16 in the G2 group (chronic respiratory disease). In both groups the peripheral muscular strength was diminished. The MRC lower members showed a negative negative correlation in the G2 group (r = -0.44) with the length of hospital stay. Respiratory muscle strength showed a decrease with statistical significance when compared to predicted (p: 0.0001). Palmar grip strength (PGS), MRC and respiratory muscle strength showed regular and strong correlations, but FIM showed no correlation with these variables Conclusion: Therefore, considering the studied sample, there was a significant reduction of peripheral muscle strength and respiratory muscle strength with statistical significance when compared to the predicted, showing significant positive and negative correlations and interferences between the variables studied in both groups groups. Introdução: As doenças respiratórias afetam milhões de pessoas no mundo sendo as causas mais frequentes de adoecimento e óbitos atingindo todas as faixas etárias principalmente crianças e idosos. No Brasil, os índices de admissão hospitalizar por doenças respiratórias ocupam o primeiro lugar no país, com grande impacto socioeconômico. Objetivo: Avaliar a força muscular e a funcionalidade de pacientes internados por doenças respiratórias agudas e crônicas agudizadas. Método: Estudo transversal, realizado em pacientes diagnosticados e internados com doenças respiratórias agudas ou crônicas agudizadas. As avaliações foram realizadas por meio da Medical Research Council (MRC), Medida de independência funcional (MIF), manovacuometria e dinamometria de preensão palmar nos pacientes internados no Hospital Universitário de Lagarto. Resultados: Foram incluídos na análise 56 pacientes, 40 pertencentes ao grupo G1 (doença respiratória aguda) e 16 no grupo G2 (doença respiratória crônica agudizada). Em ambos os grupos a força muscular periférica se apresentou diminuída. A MRC membros inferiores (MMII) demonstrou correlação regular negativa no grupo G2 (r = -0.44) com o tempo de internação. A força muscular respiratória mostrou uma diminuição com significância estatística quando comparado ao predito (p:0,0001). A força de preensão palmar (FPP), MRC e força muscular respiratória demonstraram correlações regulares e fortes, mas a MIF não demonstrou correlação com essas variáveis. Conclusão: Houve a redução significativa de força muscular periférica e da força muscular respiratória com significância estatística quando comparado com o predito, demonstrando ainda, correlações e interferências positivas e negativas significativas entre as variáveis pesquisadas em ambos os grupos.
