Relations between health related quality of life and self-concepts and significant others concepts of adolescents through structural equation modeling

From data collected in the adolescent population of four Brazilian capitals (Belém, Brasília, Fortaleza and Porto Alegre), test the validity of theoretical model that investigates the relationship between the variables involved in the constructs of the familiar identifying patterns obtained with the Identification Test Family (FIT) and those of health-related quality of life questionnaire obtained using the generic KIDSCREEN-27, by applying the structural equation modeling (SEM). 1,082 adolescents selected, constituted three groups: one of the attending health care services in outpatient mental health (CG), a student group of the public sector (GP) and a student group of the private sector (GPr). The three groups were investigated with the KIDSCREEN-27 in five dimensions of quality of life related to health (HRQoL) and the FIT (the Family Identification Test) in their patterns of family identifications. The instruments were analyzed in its psychometric properties, scores of HRQoL were obtained, self-concepts and family identifications that allowed us to compare the three groups with descriptive and inferential statistics (MANOVA, MANCOVA and Discriminante Analysis); and, in a second step, were analyzed the repercussions of identification patterns in HRQoL by structural equation modeling. The mean age of the adolescents in the general sample was from 15.30 (SD = 1.60) years old; 54.1% female; lower scores were found in adolescents with mental illness in four of five dimensions of KIDSCREEN-27, with an effect size ranging from 0.25 to physical well-being 0.46 for Autonomy & Parents. In FIT analysis, was obtained a statistically significant effect with MANOVA that indicated differences in the family identification patterns. Adolescents from PrG had higher scores than PG adolescents in self-congruence, identifications actual and ideal with the father and ideal identification with grandparents; and also showed differences with the CG in actual and ideal identification with the father. The results differentiated adolescents from different backgrounds. In the last analysis stage, was conducted a factorial structure study of the KIDSCREEN-27 and the FIT and after that was conducted a Multivariate Regression Model and it was observed that the actual self-concept predicts the quality of life in its five dimensions; the perceived image of the mother, father and grandfather / grandmother predicts the quality of life mainly in the aspect of quality of life related to Autonomy & Relationship with Parents. The study provides relevant and valuable information on the measurement of HRQoL and Identifications Patterns in Brazilian adolescents groups from different backgrounds. The finding that adolescents from public schools and clinical group show fewer desires to look like their parents can point to a lower family cohesion or more pronounced feelings of rejection with father figure. Additionally, aspects highlighted by
the perception of self-concepts and other's concepts can be predictors of higher scores of Health-Related Quality of Life. Both instruments, therefore, are useful tools in assessing the mental health and well-being of adolescents. A partir de dados coletados em população de adolescentes de quatro capitais brasileiras
(Belém, Brasília, Fortaleza e Porto Alegre), testar a validade de modelo teórico que investiga
as relações entre as variáveis envolvidas nos constructos dos padrões de identificação familiar
obtidas com o Teste de Identificação da Família (FIT) e àquelas de qualidade de vida
relacionada à saúde obtidas com o questionário genérico KIDSCREEN-27, por meio da
aplicação da modelagem de equações estruturais (SEM). 1.082 adolescentes selecionados,
constituiram três grupos: um dos que frequentavam serviços de atenção em saúde mental
ambulatorial (GC), um de estudantes do setor público (GP) e um de estudantes do setor
privado (GPr). Os três grupos foram investigados com o KIDSCREEN-27, em cinco
dimensões de sua qualidade de vida relacionada à saúde (QVRS) e com o FIT (Teste de
Identificação da Familia), em seus padrões de identificações familiares. Os instrumentos
foram analisados em suas propriedades psicométricas, obtiveram-se escores de QVRS, de
autoconceitos e de identificações familiares que permitiram comparar os três grupos com
estatísticas descritivas e inferenciais (MANOVA, MANCOVA e Análise Discriminate); e em
uma segunda etapa, foram analisadas as repercussões dos padrões de identificação na QVRS
por meio da Modelagem de Equações Estruturais. A média de idades dos adolescentes na
amostra geral foi de 15.30 (SD = 1.60) anos, sendo 54,1% do sexo feminino; foram
encontrados escores mais baixos de QVRS em adolescentes com doença mental em quatro de cinco dimensões, com um tamanho de efeito variando de 0,25 para bem-estar físico a 0,46 para a Autonomia & Pais. Na análise do FIT, foi obtido um efeito estatisticamente significativo com MANOVA seguido por uma série de ANOVAs que indicou diferenças nos padrões de identificação da família. Os adolescentes GPr apresentaram médias mais elevadas do que adolescentes de GP em auto-congruência, identificações real e ideal com o pai e identificação ideal com avós; e também mostrou diferenças com o GC na identificação real e ideal com o pai. Os resultados diferenciaram adolescentes oriundos de contextos diferentes. Na última etapa de análise, foi realizado estudo da estrutura fatorial do KIDSCREEN-27 e do FIT e, a seguir, foi realizado um Modelo Multivariado de Regressão Linear e foi observado que o autoconceito real prevê a qualidade de vida em suas cinco dimensões; a imagem percebida da mãe, do pai e do avô/avó prevê a qualidade de vida principalmente na dimensão da qualidade de vida relacionada a Autonomia & Relação com os Pais. O estudo fornece informações pertinentes e valiosas sobre a mensuração de QVRS e Padrões de Identificações em grupos de adolescentes brasileiros de diferentes origens. A constatação de que adolescentes oriundos de escolas públicas e grupo clínico mostrarem um desejo menor para se parecerem como seus pais pode apontar para uma menor coesão familiar ou sentimentos mais pronunciados de rejeição com figura paterna. Além disso, aspectos destacados pela percepção de autoconceitos e conceitos de outros podem ser preditores de escores maiores de qualidade de vida relacionada à saúde. Ambos os instrumentos constituem, portanto, ferramentas úteis na avaliação da saúde mental e bem-estar de adolescentes.
