| dc.description | As Unidades de Terapia Intensiva (UTIs), a partir da utilização de tecnologia sofisticada e emprego de metodologias criadas para aferir e controlar as variáveis vitais, são responsáveis pela manutenção e prolongamento da vida, por vezes, além do desejado ou indicado para certos pacientes. Frente a real possibilidade de adiar o momento da morte, cada vez mais as equipes de saúde da UTI se deparam com questões éticas, morais, religiosas e até judiciais, que tornam o trabalho realizado neste setor por vezes difícil, complexo e desgastante. A grande dúvida para estes profissionais parece ser até quando investir em tratamentos e quando interrompê-los. O presente estudo procurou conhecer, através de entrevistas e observação participante, como os médicos intensivistas responsáveis pela rotina de uma UTI lidam com questões por eles mesmos qualificadas como dilemas e identificar os aspectos da organização de trabalho que, por ventura, facilitam ou dificultam este enfrentamento. Os resultados revelam a angústia do médico intensivista em tomar decisões solitariamente e ao mesmo tempo a dificuldade na criação de espaços de discussão e compartilhamento das condutas. Questões como a relação dos médicos com as famílias dos pacientes, a interação com outros membros da equipe, as inadequações da formação médica para lidar com a morte foram discutidas, bem como as motivações e perspectivas profissionais futuras. | |
| dc.description | Intensive Care Units (ICUs), based on the use of sophisticated technology and employment of methods designed to assess and control vital variables, are responsible for the maintenance and extension of life, sometimes beyond the desired or indicated for certain patients. Due to the real possibility to postpone the moment of death, health professional teams are faced with ethical, moral, religious and even legal issues, which make the work sometimes difficult, complex and wearing. The main question for these professionals seems to be when investing in treatments and when to interrupt them. The present study sought to examine, through interviews and participant observation, how medical doctors responsible for the routine of a UIC deal with issues classified by themselves as dilemmas and identify aspects of work organization that might facilitate or hinder this confrontation. The results reveal the anguish of doctors in making decisions by themselves, and, on the other hand, the difficulty in creating spaces for discussion and sharing medical problems. The relationship with patients´ families, the interaction with other members of the team, the inadequacy of medical training to deal with death were also discussed, as well as motivations and future professional prospects. | |
