| dc.description | Este estudo trata aspectos específicos da implantação da Política de Educação Permanente em Saúde (PNEPS) no estado do Rio de Janeiro. A partir da realização de entrevistas semi-estruturadas com atores-chave, foi analisado o processo decisório de formação e funcionamento dos cinco Pólos de Educação Permanente em Saúde (PEPS) constituídos. Considerando a Portaria 198 GM/MS de 2004, a análise se deu a partir de dois grandes eixos: a participação na gestão colegiada e a operacionalização da Política. Na implementação dos PEPS no estado destaca-se a ruptura com a proposta anterior dos Pólos de Capacitação para o Pessoal do PSF e a atuação da Secretaria Estadual de Saúde (SES) como articuladora do processo. Conforme previsto pela Portaria, os PEPS se configuraram como Colegiados de Gestão, instância locorregional/interinstitucional com representação diversificada. Procedeu-se neste estudo a análise da atuação dos atores, os interesses identificados e as relações de poder existentes na roda de gestão. Apesar da participação de vários segmentos importantes observou-se uma deficiente representação dos gestores da saúde e dificuldades no exercício do controle social pelos representantes dos conselhos de saúde. Embora se observe importante adesão a Política, a falta de financiamento dos projetos elaborados foi a principal causa do desinteresse dos participantes que resultou num esvaziamento das reuniões a época do estudo. A hierarquia existente no fluxo de encaminhamento dos projetos entre as instâncias locorregionais, o estado e o MS, acarretou grande atraso na tramitação das propostas. Por fim, compreende-se que o processo de implementação da Política não considerou os arranjos institucionais existentes nos três níveis da federação e sua relevância como determinantes do curso da mesma. | |
| dc.description | This work approaches specific aspects regarding the implementation of the Policy for Permanent Education in Health (PNEPS) in the state of Rio de Janeiro. Based on semistructured interviews with key-actors, the decision-making process of formation and functioning of the five Poles of Permanent Education in Health (PEPS) constituted were analyzed. Considering the governmental decree 198 GM/MS from 2004, the analysis was conducted from two main axes: the participation on the collegiate management and the practical functioning of the Policy. Regarding the implementation of the PEPS on the state, the rupture with the previous proposal of Training Poles for Health Family Program (PSF) Staff and the action of the State Health Secretariat (SES) as articulating member of the process, are highlighted. As stated at the government decree, the PEPS are configured as Management Collegiate, a local-regional/inter-institutional instance with diversified representation. In this study, an analysis of the actors’ role, other identified interested actors and the power relationships existent around the management wheel was performed. Despite the participation of various important segments, it was observed that there was a deficient representation of health managers, as well as difficulties on the exercising of social control by the representatives of the health councils. Although an important adhesion to the Policy is observed, the lack of financial support to the elaborated projects was the main cause of disinterest from the participants, which resulted on deserted meetings at the time of the study. The present hierarchy found on the flow of direction for projects among local-regional and state instances and the Ministry of Health, has resulted in a large delay on the course of the proposals. Finally, it becomes clear that the implementation process of the Policy did not consider the institutional arrangements existent throughout the three levels of government and their relevance as determinant aspects on the Policy’s course. | |
