Resumen
O Programa de Saúde da Família (PSF) deu lugar à crescente expansão da Estratégia Saúde da Família (ESF) no país e ao seu caráter organizador. Entretanto, essa mesma expansão influenciou uma tendência de criação e de fortalecimento de estruturas regulatórias, resultando em uma série de medidas de avaliação da qualidade dos efeitos da a APS. Neste sentido, este estudo visou discutir a organização administrativa da gestão da saúde e analisar indicadores de Internação Hospitalar, em Teresina-PI, no período de 2008 a 2020. Para tanto, levantou-se as principais mudanças organizacionais e de fluxos, na Fundação Municipal de Saúde, para buscar compreender efeitos nos serviços e em indicadores; ao passo que, foram analisados dados do Sistema de Gerenciamento de Internações Hospitalares, desta capital, para caracterizar a evolução de internações, no período do estudo, por meio de análises estatísticas descritivas e multivariadas. Sobre os resultados, foram demonstradas e problematizadas as principais características dos distintos arranjos organizacionais da saúde local ao longo dos anos estudados, tendo sido verificado recorrente falta de discussão e planejamento da gestão local, resultando em reformas administrativas que se resumiram à reestruturação burocrática necessária diante do novo modelo assistencial do Sistema Único de Saúde (SUS). De outro lado, a análise de dados de Internações Hospitalares, possibilitou uma visão sistémica quanto ao perfil das Internações por Condições Sensíveis à Atenção Primária (CSAP), enquanto o desenvolvimento de um modelo estatístico propiciou melhora na produção de informações mais assertivas de residentes e não residentes internados. Desta forma, verificou-se mais chances de homens estarem internados por CSAP, especialmente os indivíduos mais velhos, pois apresentaram pior desempenho. Ainda, que quanto as CSAP, o custo predominante das internações foi o de valores mais baixos; e que, sobre o período de permanência, quanto menor foi o período de internação, maiores foram as chances de tratar-se de CSAP. Ademais, apesar de não verificar-se correlacões entre a cobertura da Atenção Primária e as ocorrências de internações por CSAP, compreende-se que os gestores podem atuar neste cenário monitorando morbidade hospitalar e realizando avaliações da assistência prestada usando ferramentas de análises destas internações para alcançar melhoria da qualidade e maior eficiência operacional. The Family Health Program (PSF) gave way to the growing expansion of the Family Health Strategy (ESF) in the country and its character organizer. However, this same expansion influenced a trend towards the creation and strengthening of regulatory structures, resulting in a series of measures to assess the quality of its attributes. In this sense, this study aimed to discuss the administrative organization of health management and analyze Hospital Admission indicators, in Teresina-PI, from 2008 to 2020. To this end, the main organizational and flow changes were raised at the Municipal Health Foundation, in order to understand the effects on services and indicators; whereas, data from the Hospital Admission Management System, in this capital, were analyzed to characterize the evolution of admissions, during the study period, through descriptive and multivariate statistical analyses. Regarding the results, the main characteristics of the different organizational arrangements of local health over the years studied were demonstrated and problematized, with a recurrent lack of discussion and planning of local management, resulting in administrative reforms that were summarized in the necessary bureaucratic restructuring in light of the new care model of the Unified Health System (SUS). On the other hand, the data analysis of Hospital Admissions allowed a systemic view of the profile of Admissions for Sensitive Conditions to Primary Care (CSAP), while the development of a statistical model provided an improvement in the production of more assertive information for residents and not interned residents. Thus, there was more chances of men being hospitalized for ACSC, especially older individuals, as they showed worse performance. Still, as for CSAP, the predominant cost of hospitalizations was the lowest; and that, regarding the period of stay, the shorter the period of hospitalization, the greater the chances of dealing with ACSC. Furthermore, although there is no correlation between the coverage of Primary Care and the occurrences of hospitalizations for ACSC, it is understood that managers can act in this scenario by monitoring hospital morbidity and carrying out assessments of the care provided using analysis tools of these admissions to achieve improved quality and greater operational efficiency.
