Purpose: Describe the effect of the diode laser with a wavelength of 650nm in the management of pain in teeth with a diagnosis of symptomatic apical periodontitis, treated in the endodontics postgraduate program of the Universidad Santo Tomás. Materials and methods: This study included teeth with symptomatic apical periodontitis from patients who attended between August 2022 and May 2023, who underwent endodontics and low power laser therapy in a single-visit. Pain intensity was measured on a combined scale at 8, 24, 48, 48, 72 hours, 8, 15 and 30 days. Results: 3 cases were evaluated, no patient reported taking medication as an adjuvant in the management of postoperative pain, the patients reported preoperative pain between unbearable-very severe, at 8 hours it decreased to moderate-strong and at 24 hours moderate-little pain, one patient reported no pain at 48 hours, and the other two patients up to 72 hours reported no pain. In the clinical assessment at 8 days no patient reported pain to percussion and/or palpation. Conclusions: LLLT with a wavelength of 650 nm showed decreased postoperative pain and reduced analgesic consumption.
