Purpose: Evaluate biometric ocular parameters, refractive errors and eye growth in a series of premature infants (birth weight under 1,500 g or gestational age less than 32 weeks) with and without retinopathy of prematurity (ROP). Methods: Prospective study in preterm population performed in the pediatric intensive care unit of Hospital São Paulo, Department of Pediatrics, UNIFESP, during the years 2015-2017. Follow-up was fortnightly considering the following intervals of gestational age in weeks: 26-27; 28-29; 30-31; 32-33; 34-35; 36-37; 38-39 and 40-41. Biometric Ocular parameters were obtained using transpalpebral (B-scan ultrasound, 7.5-15 MHz linear transducer, Mylab, Esaote) and included: Anterior chamber depth, lens thickness, vitreous chamber depth and axial length. Ophthalmological exam included refraction (retinoscopy, calculated spherical equivalent) and indirect ophthalmoscopy (28 D lens), screening for ROP. Pediatric parameters included: gestacional age corrected in weeks, weight in grams, head circumference, weight, birth length and Apgar score. Results: 60 eyes of premature newborn were included and analyzed the right eye of 42 premature babies (20 male, 22 female). Through repeated analysis of variance (ANOVA), a statistically significant effect was observed over time, for the studied ocular parameters, and with regard to retinopathy of prematurity, it did not differ significantly from any of the measures evaluated from the rest of the group.The biometric parameter showed a positive correlation relationship with gestacional age, however, the refractive spherical equivalent was observed in an opposite behavior. The lens thickness and refractive spherical equivalent did not show significant correlation with gestational age, only the spherical equivalent showed significant correlation with the z-weight score for gestational age. Conclusions: In this study, it was possible to document the pattern of ocular growth in preterm infants with the progression of gestational age, without differentiation between the groups of preterm infants (with and without retinopathy of prematurity) by validating mode B ultrasound with the eyelid contact technique. Objetivo: Avaliar os parâmetros oculares biométricos, erros de refração e crescimento ocular em um série de bebês prematuros (peso ao nascer menor que 1.500 g ou idade gestacional menor que 32 semanas) com e sem retinopatia da prematuridade (ROP). Métodos: Estudo prospectivo na população pré-termo realizada na unidade de terapia intensiva pediátrica do Hospital São Paulo, Departamento de Pediatria, UNIFESP, durante os anos 2015-2017. Acompanhamento foi quinzenal considerando os seguintes intervalos de idade gestacional em semanas: 26-27; 28-29; 30-31; 32-33; 34-35; 36-37; 38-39 e 40-41. Parâmetros oculares biométricos foram obtidos usando transpalpebral (ultrassom B-scan, transdutor linear de 7,5-15 MHz, Mylab, Esaote) e incluídos: profundidade da câmara anterior, espessura da lente, vítreo profundidade da câmara e comprimento axial. Exame oftalmológico incluiu refração (retinoscopia, equivalente esférico calculado) e oftalmoscopia indireta (28 D lente), triagem para ROP. Parâmetros pediátricos incluídos: idade gestacional corrigida em semanas, peso em gramas, perímetro cefálico, peso, comprimento ao nascer e índice de Apgar. Resultados: 60 olhos de recém-nascidos prematuros foram incluídos e analisados o olho direito de 42 bebês prematuros (20 homens, 22 mulheres). Por meio de análises repetidas de variância (ANOVA), um efeito estatisticamente significativo foi observado ao longo do tempo, para a ocular estudada parâmetros, e no que diz respeito à retinopatia da prematuridade, não diferiu significativamente de qualquer uma das medidas avaliadas do resto do grupo. O parâmetro biométrico apresentou relação positiva com a idade gestacional, porém, o fator refrativo equivalente esférico foi observado em um comportamento oposto. A espessura da lente e equivalente esférico refrativo não mostrou correlação significativa com a idade gestacional, apenas o equivalente esférico mostrou correlação significativa com a pontuação de peso z para idade gestacional. Conclusões: Neste estudo, foi possível documentar o padrão do crescimento ocular em prematuros com a progressão da idade gestacional, sem diferenciação entre os grupos de bebês prematuros (com e sem retinopatia de prematuridade) validando o ultrassom modo B com a técnica de contato palpebral.
