Integração ensino-serviço-comunidade no município de Bragança Paulista - SP a partir do contrato organizativo de ação pública ensino-saúde

Produto de uma ação conjunta dos Ministérios da Saúde e da Educação, o Contrato Organizativo de Ação Pública Ensino-Saúde (COAPES) visa fortalecer a integração ensino-serviço-comunidade (IESC) e, como dispositivo da Política Nacional de Educação Permanente em Saúde pretende ser capaz de promover processos participativos de construção da formação e desenvolvimento profissional no/para o SUS. Com o objetivo de compreender como a IESC tem se desenvolvido no município de Bragança Paulista/SP a partir do Contrato Organizativo de Ação Pública Ensino-Saúde, cuja primeira contratualização se deu em 2016, realizou-se uma pesquisa de estudo de caso, com abordagem qualitativa, descritiva e exploratória, em duas etapas: Pesquisa Documental e Pesquisa de Campo, em que foram identificadas as diretrizes relacionadas à IESC em documentos originais de abrangência nacional e nos documentos construídos com base na realidade local e a partir de entrevistas semiestruturadas com dez membros do Comitê Gestor Local do COAPES, envolvidos diretamente na operacionalização da IESC no município. À semelhança da maior parte do país, a efetiva integração entre o ensino, serviço e comunidade ainda não foi consolidada no território. Todavia, foram identificados avanços importantes. A regulação de acesso dos estudantes da área da saúde aos cenários de prática é um processo estabelecido, que conta com mapeamento da rede de saúde para organizar o desenvolvimento das ações. O estabelecimento de relações dialógicas foi considerado uma conquista do COAPES, sendo que a comunicação se fortaleceu e novos projetos e posturas foram constituídos. Dentre as lacunas existentes, a falta de elementos indicativos de que a Educação Permanente em Saúde é uma prática cotidiana, a estrutura física deficiente, as práticas pedagógicas pouco eficientes e a falta de alinhamento entre o ensino e saúde no que tange a preceptoria, se mostraram como potentes desafios. A ausência de registros das ações realizadas e das estratégias utilizadas pelo Comitê Gestor Local representou limitações à pesquisa. Assim, acredita-se que a revisão de sua estrutura e funcionamento, a propositura de estratégias para o avanço do monitoramento e avaliação das ações desenvolvidas, a redução de contradições, o fortalecimento da Educação Permanente em Saúde e a manutenção do compromisso com as responsabilidades mútuas e colaborativas sejam as principais metas para a sustentabilidade e avanços dessa contratualização. Contudo, são consideradas assertivas as estratégias utilizadas na condução da IESC e do COAPES no município de Bragança Paulista – SP, uma vez que se pautaram na aproximação, diálogo institucional, igualdade de valor e transparência. Product of a joint action by the Ministries of Health and Education, the TeachingHealth Public Action Organizational Contract (COAPES) aims to strengthen the
teaching-service-community integration (IESC) and, as a device of the National
Policy on Permanent Education in Health, intends to be able to promote participatory
processes of building training and professional development in/for the SUS. With the
aim of understanding how the IESC has developed in the municipality of Bragança
Paulista/SP from the Public Action Organizational Contract Teaching-Health, whose
first contractualization took place in 2016, a case study research was carried out, with
an approach qualitative, descriptive and exploratory, in two stages: Documentary
Research and Field Research, in which guidelines related to the IESC were identified
in original documents of national scope and in documents constructed based on the
local reality and from semi-structured interviews with ten members of the Local
Management Committee of COAPES, directly involved in the operationalization of the
IESC in the municipality. As in most of the country, the effective integration between
teaching, service and community has not yet been consolidated in the territory.
However, important advances were identified. The regulation of health students'
access to practice scenarios is an established process, which includes mapping of
the health network to organize the development of actions. The establishment of
dialogic relationships was considered an achievement of COAPES, and
communication was strengthened, and new projects and postures were constituted.
Among the existing gaps, the lack of elements indicating that Permanent Education
in Health is a daily practice, the deficient physical structure, the inefficient
pedagogical practices, and the lack of alignment between teaching and health in
terms of preceptorship, were shown to be as powerful challenges. The absence of
records of the actions carried out and the strategies used by the Local Management
Committee represented limitations to the research. Thus, it is believed that reviewing
its structure and functioning, proposing strategies to advance the monitoring and
evaluation of the actions developed, reducing contradictions, strengthening
Permanent Education in Health, and maintaining the commitment to mutual
responsibilities and collaborative are the main goals for the sustainability and
advances of this contractualization. However, the strategies used in conducting IESC
and COAPES in the municipality of Bragança Paulista/SP are considered assertive,
since they were based on approximation, institutional dialogue, equal value, and
transparency.
