Disfunção endotelial e estado nutricional relativo ao selênio em pacientes pediátricos durante a fase aguda da resposta inflamatória sistêmica

The association between endothelial adhesion molecules and selenium has been shown experimentally, but clinical studies have not clarified whether selenium status has an active role in endothelial function. The objectives of this study were to investigate whether there is an association between erythrocyte selenium and (a) the magnitude of endothelial activation and (b) the severity of multiple organ dysfunction in children during the Systemic Inflammatory Response Syndrome (SIRS). A prospective cohort study was carried out in 109 children with SIRS admitted to a pediatric ICU. Erythrocyte and plasma selenium were measured on admission and Selenoprotein P and soluble plasma forms of Intercellular Adhesion Molecules 1 (sICAM-1), Vascular Cell Adhesion Molecule 1 (sVCAM-1), sPselectin and EndoCAM on days 1, 2 and 3. ANOVA for repeated measures were adjusted for clinical severity paremeters, C-reactive protein, procalcitonin and serum lactate. The effect of selenium on organ dysfunction was defined by the Pediatic Logistic Organic Dysfunction (PELOD-2) during ICU stay. Erythrocyte selenium was associated with sPselectin and EndoCAM: an increase of 10 μmol/dL resulted in increases of 43.2 ng/mL (95% CI: 17.2; 69.2, p = 0.001) in sP-selectin and of 0.04 ng/mL (95% CI: 0.02; 0.06, p <0.001) in EndoCAM. Erythrocyte selenium was associated with a decrease in PELOD-2 (β: -0.3; 95% CI: -0.5; -0.06, p=0.015). Plasma selenium was not related to any of the outcomes. Erythrocyte selenium is associated with endothelial activation that occurs in children in the early phase of SIRS and has a possible protective effect on organ dysfunction. Os objetivos deste estudo foram investigar se existe uma associação entre o selênio eritrocitário e (a) a magnitude da ativação endotelial e (b) a gravidade da disfunção de múltiplos órgãos em crianças durante o período sistêmico. Síndrome da Resposta Inflamatória (SIRS). Um estudo de coorte prospectivo foi realizado em 109 crianças com SIRS internadas em uma UTI pediátrica. O selênio eritrocitário e plasmático foram medidos na admissão e as formas de Selenoproteína P e plasma solúvel de Moléculas de Adesão Intercelular 1 (sICAM-1), Molécula de Adesão de Células Vasculares 1 (sVCAM-1), sPselectina e EndoCAM nos dias 1, 2 e 3. ANOVA para medidas repetidas foram ajustadas para parâmetros de gravidade clínica, proteína C reativa, procalcitonina e lactato sérico. O efeito do selênio na disfunção orgânica foi definido pela Disfunção Orgânica Logística Pediátrica (PELOD-2) durante a permanência na UTI. O selênio eritrocitário foi associado à sPselectina e EndoCAM: um aumento de 10 μmol/dL resultou em aumentos de 43,2 ng/mL (95% IC: 17,2; 69,2, p = 0,001) em sP-selectina e de 0,04 ng/mL (95% CI: 0,02; 0,06, p <0,001) em EndoCAM. O selênio eritrocitário foi associado à diminuição do PELOD-2 (β: -0,3; IC 95%: -0,5; -0,06, p=0,015). O selênio plasmático não foi relacionado a nenhum dos desfechos. O selênio eritrocitário está associado à ativação endotelial que ocorre em crianças na fase inicial da SIRS e tem um possível efeito protetor na disfunção orgânica.
