Infiltração intra-articular de corticoide X ozônio na ATM: uma revisão de literatura

musculoesqueléticos que descrevem uma série de problemas clínicos que afetam os músculos
da mastigação, a articulação temporomandibular (ATM) e as suas estruturas associadas. O
objetivo desta revisão de literatura foi elaborar uma revisão anatômica da ATM e as disfunções
que podem acometê-la, bem como realizar uma revisão de literatura a fim de comparar a
infiltração de corticoide versus a infiltração de ozônio na ATM para o tratamento das DTMs
articulares dolorosas. Para tal, foi realizado um levantamento bibliográfico nas principais bases
de dados da saúde: PubMed, Scielo, Biblioteca Virtual em Saúde, LILACS, Web of Science e
Embase. Utilizou-se os descritores DeCs e Mesh: “disfunção temporomandibular”, “artralgia”,
“ozônio”, “corticosteroides”, “injeções intra-articulares”, “infiltração”, “terapia
medicamentosa” adaptados para cada base de dados. Foram incluídos artigos de revisão
sistemática, estudos observacionais e estudos de intervenção publicados nos últimos 20 anos,
sem haver limitação de idioma, faixa etária ou tipo de população. Foram excluídos artigos em
duplicata, fuga ao tema proposto e indisponibilidade para leitura integral. Como tratamento das
DTMs articulares, a infiltração intra-articular de corticosteroides apresenta alto poder antiinflamatório local. Uma nova opção terapêutica é a infiltração de gás ozônio que tem
demonstrado, nos poucos estudos disponíveis, sua capacidade de regulação da resposta
imunológica, com efeitos anti-inflamatórios e analgésicos. Analisando os resultados obtidos
por meio da presente revisão, pode-se observar certa semelhança na eficácia de ambos os
tratamentos quanto à diminuição dos sintomas álgicos articulares. Contudo, a ozonioterapia
apresenta ligeira vantagem quanto à duração do efeito analgésico e à diminuição das possíveis
complicações que podem ocorrer. Portanto, foi possível concluir que ambos os tratamentos são
eficientes no controle da DTM articular dolorosa, e que a infiltração intra-articular de ozônio
se mostra como um tratamento promissor, porém requer maior aprofundamento científico. Temporomandibular disorders (TMD) are a subclassification of musculoskeletal disorders that
describe a series of clinical problems affecting the masticatory muscles, temporomandibular
joint (TMJ) and its associated structures. The purpose of this literature review was to elaborate
an anatomical review of the TMJ and the dysfunctions that can affect it, as well as to conduct a
literature review in order to compare corticoid infiltration versus ozone infiltration of the TMJ
for the treatment of painful joint TMD. To this end, a bibliographic survey was carried out in
the main health databases: PubMed, Scielo, Virtual Health Library, LILACS, Web of Science
and Embase. The DeCs and Mesh descriptors were used: "temporomandibular dysfunction",
"arthralgia", "ozone", "corticosteroids", "intra-articular injections", “infiltration”, "drug
therapy", adapted for each database. Systematic review articles, observational studies, and
intervention studies published in the last 20 years were included, with no limitation on language,
age range, or type of population. Duplicate articles, articles that were unrelated to the proposed
topic, and articles that were unavailable to be read in full were excluded. As a treatment of
TMD, the intra-articular infiltration of corticosteroids have a high local anti-inflammatory
power. A new therapeutic option is ozone gas infiltration, which has demonstrated, in the few
studies available, its high capacity to regulate the immune response, with anti-inflammatory
and analgesic effects. Analyzing the results obtained in this review, we can see a certain
similarity in the efficacy of both treatments regarding the reduction of joint pain symptoms.
However, ozone therapy has a slight advantage regarding the duration of the analgesic effect
and the reduction of possible complications that may occur. Therefore, it was possible to
conclude that both treatments are efficient in the control of painful TMD, and that intra-articular
ozone infiltration is a promising treatment, but requires further scientific study.
