Relación entre el consumo de alcohol y cáncer de mama según la expresión de receptores hormonales en mujeres de tres ciudades colombianas

RESUMEN: Antecedentes La literatura reporta la relación que hay entre la ingesta de alcohol con el desarrollo de cáncer de mama en mujeres alrededor del mundo y que esta relación además puede variar de acuerdo con el estado de receptores hormonales (estrógeno y progesterona). En mujeres colombianas, son pocos los estudios en los que se ha estudiado dicha relación.
Metodología Se realizó un estudio poblacional de casos y controles en mujeres de 22 a 86 años en tres ciudades de Colombia. Se reclutaron 316 casos con diagnóstico de cáncer de mama primario, confirmado por biopsia invasivo o in situ y 316 controles pareados por edad y área de residencia. A cada participante se le aplicó un cuestionario para evaluar los factores de riesgo para cáncer de mama, entre ellos el consumo de alcohol durante toda la vida. La inmunohistoquímica se determinó de manera centralizada por el Centro Fred Hutchinson de Investigación en Cáncer (FHCRC) en Seattle WA (90 Casos) y por laboratorios nacionales de Colombia (173 casos). Se usaron modelos de regresión logística para estimar los ORs con IC 95%.
Resultados Se encontró una asociación inversa entre el consumo actual (OR: 0.59, IC 95%: 0.41-0.85, p < 0.05), los gramos de alcohol por día consumidos de los 12 a los 18 años (OR: 0.54, IC 95%: 0.3-0.95, p < 0.05) con el cáncer de mama y también para el consumo de 2 gramos o más de alcohol durante toda la vida en mujeres con tumores receptor hormonal ER+/PR- (OR: 0.2, IC 95%: 0.04-0.99, p < 0.05).
Conclusiones Nuestros hallazgos indican que el consumo de alcohol en la población colombiana tiene una tendencia de protección para desarrollar cáncer de mama, por lo tanto, nuestros resultados no concuerdan con lo reportado frecuentemente en la literatura; no obstante, es comparable con estudios realizados en población asiática.
