Características sociodemográficas, clínicas y microbiológicas de los pacientes con neumonía por Pneumocystis jirovecii con coinfecciones pulmonares: Una revisión sistemática - reporte preliminar

RESUMEN:
Introducción: Actualmente la información existente acerca de las coinfecciones pulmonares con P. jirovecii se encuentra dispersa en la literatura, principalmente en reportes de casos. A la fecha no existe ninguna revisión sistemática sobre este tema, por lo que existe la necesidad de consolidar la información disponible para aproximarnos a conocer si existe alguna particularidad con este grupo de pacientes.
Objetivo: Determinar las características sociodemográficas, clínicas y microbiológicas de los pacientes con neumonía por P. jirovecii que tienen coinfecciones pulmonares reportados en la literatura a nivel mundial.
Materiales y métodos: Revisión sistemática de series y reportes de casos de coinfecciones pulmonares con P. jirovecii, se emplearon 30 estrategias de búsqueda en 6 bases de datos multidisciplinarias. Los criterios de inclusión y calidad de los artículos se evaluaron según la guía CARE por los tres autores. Las variables se
tabularon para cada paciente y se hizo un análisis descriptivo univariado con el software SPSS.
Resultados: Con la estrategia de búsqueda planteada se obtuvieron 78.820 artículos, y finalmente se seleccionaron 74 artículos que cumplían los criterios de inclusión, 64 de ellos cumplían los criterios de calidad definidos basados en la guía CARE. Para este reporte parcial, 52 fueron reportes de caso y 12 series de casos, a partir de estos obtuvimos información de 72 pacientes con PCP y coinfección pulmonar. Se reportaron más casos en hombres (63.9%), un 68.1% son casos en adultos jóvenes, y un 37.5% son VIH, sin embargo, son frecuentes en los reportes otro tipo de patologías de base. El 70% tienen un copatógeno, pero hay reporte de hasta 4 patógenos al tiempo. La coinfección más común fue con Citomegalovirus (35.8%), seguido de Aspergillus fumigatus (10.4%). El método diagnóstico más común de la PCP fue la microscopía (56%).
Conclusiones: De acuerdo a la información encontrada, las características sociodemográficas y clínicas de los pacientes con copatógenos son similares a las conocidas para la PcP en infección única. ABSTRACT:
Introduction: Currently the existing information about pulmonary coinfections with P. jirovecii is scattered in the literature, mainly in case reports. Up to this date, there is no systematic review on this topic, so there is a need to consolidate the information available to approach to know if there is any particularity with this group of patients.
Objective: To determine the sociodemographic, clinical and microbiological characteristics of patients with P. jirovecii pneumonia who have pulmonary co-infections reported in the literature worldwide.
Materials and methods: Systematic review of series and case reports of pulmonary coinfections with P. jirovecii, 30 research strategies were used in 6 multidisciplinary databases. The inclusion criteria and quality of the articles were evaluated according to the CARE guideline by the three authors. The variables were tabulated for each patient and a univariate descriptive analysis was made with the SPSS software.
Results: With the proposed research strategy, 78,820 articles were obtained, and finally 74 articles were selected that met the inclusion criteria, and 64 of them met the quality criteria defined based on the CARE guideline. For this partial report, 52 were case reports and 12 case series, from these we obtained information on 72 patients with PCP and pulmonary coinfection. More cases were reported in men (63.9%), 68.1% are cases in young adults, and 37.5% are HIV, however, other types of underlying pathologies are frequent in the reports. 70% have a co-pathogen, but there are reports of up to 4 pathogens at the same time. The most common coinfection was with Cytomegalovirus (35.8%), followed by Aspergillus fumigatus (10.4%). The most common diagnostic method for PCP was microscopy (56%).
Conclusions: According to the information found, the sociodemographic and clinical characteristics of patients with copathogens are similar to those known for PcP in a single infection.
