Caracterización de pacientes en la unidad de cuidados intensivos con ataque cerebrovascular isquémico sometidos a terapia de reperfusión

RESUMEN: Objetivo: Caracterizar los pacientes con ataque cerebrovascular isquémico (ACV), sometidos a alguna terapia de reperfusión, con posterior vigilancia en la UCI y variables relacionadas con el estado neurológico desfavorable al alta. Materiales y métodos: Estudio retrospectivo, observacional, analítico en pacientes mayores de edad con ataque cerebrovascular isquémico, llevados a terapia de reperfusión (trombólisis sistémica, trombectomía mecánica o ambas) entre agosto de 2019 y agosto de 2021, con posterior traslado a la unidad de cuidados intensivos para su vigilancia. Resultados: Se estudiaron 65 pacientes con edad promedio de 70 +/- 12 años, 50.8% de sexo masculino. El 66.2% fueron hipertensos, 36.9% diabéticos y el 29.2% tenían fibrilación auricular o antecedente de tabaquismo. La mediana de días de estancia en UCI/UCE fue de 3 días (RIQ: 2-4). El 10.8% requirió ventilación mecánica invasiva, el 4.6% traqueostomía y el 10.7% presentaron infección. El 18.4% presentaron sangrado en SNC, ninguno fue llevado a drenaje quirúrgico. El 20% presentaron edema cerebral, 10,7% recibieron manitol o solución salina hipertónica como medida anti-edema, 3,1% recibieron esteroides, 14 pacientes (21.5%) fallecieron. Hubo relación entre el edema cerebral (OR 20.8; IC 95%: 2.5-173, p <0.001), infección en UCI (p=0.004) y la ventilación mecánica (p=0.004) con el estado neurológico desfavorable al alta. Conclusión: El tratamiento idóneo del ACV isquémico son las terapias de reperfusión. El periodo de tiempo desde la administración de alguna de estas terapias es importante en la identificación y tratamiento de complicaciones potenciales. ABSTRACT: Objective: To characterize patients with ischemic stroke who underwent reperfusion therapy, with subsequent surveillance in the ICU and variables related to unfavorable neurological status to discharge. Materials and methods: Retrospective, observational, analytical study in elderly patients with ischemic stroke, who underwent reperfusion therapy (systemic thrombolysis, mechanical thrombectomy, or both) between August 2019 and August 2021, with subsequent transfer to the ICU for monitoring. Results: 65 patients with average age of 70 +/- 12 years, 50.8% male. 66.2% had history of hypertension, 36.9% diabetic, 29.2% had atrial fibrillation or a history of smoking. The median number of days of stay in the ICU was 3 days (IQR: 2-4). 10.8% required invasive mechanical ventilation, 4.6% tracheostomy, and 10.7% had infection. 18.4% presented bleeding in the CNS (central nervous system), none was taken to surgical drainage of the hematoma. 20% presented cerebral edema, 10.7% received mannitol or hypertonic saline as an anti-edema measure, 3.1% received steroids, 14 patients (21.5%) died. A relationship was found between cerebral edema (OR 20.8; 95% CI: 2.5-173, p <0.001), infection in the ICU (p=0.004) and mechanical ventilation (p=0.004) with unfavorable neurological status to high. Conclusion: The treatment of ischemic stroke is reperfusion therapy. The period after the administration of any of these therapies is of vital importance since potential complications can be identified and treated.
